摘 要:目的探讨MRI不同Dixon技术对成人非酒精性脂肪性肝病(NAFLD)肝脏脂肪定量的可行性。方法选取符合纳入标准的NAFLD组54例,对照组21例。两组患者均于上腹部CT检查1周内行3.0T MR VIBE Dixon检查。测量并记录CT肝/脾比值、2E-VIBE Dixon(双回波磁共振水脂分离技术)脂肪分数HFF值、6E-VIBE Dixon(6回波磁共振水脂分离技术)质子密度脂肪分数PDFF值、R 2*值。对两位医师测量的一致性和可重复性、2E-VIBE Dixon和6E-VIBE Dixon对各组肝脏脂肪定量组间及组内的差异性等进行统计学分析。结果在肝脏脂肪定量测量者之间,2E-VIBE Dixon和6E-VIBE Dixon均有高度的一致性(ICC:0.946~0.993)和可重复性(LOA:1.43%~3.72%),其中6E-VIBE Dixon更好。2E-VIBE Dixon、6E-VIBE Dixon肝脏脂肪定量在各组间和组内均具有统计学差异(P<0.05);2E-VIBE Dixon所测HFF均高于6E-VIBE Dixon所测PDFF(P<0.05)。结论2E-VIBE Dixon、6E-VIBE Dixon对NAFLD肝脏脂肪定量均具有较高的一致性和可重复性,均可用于肝脏脂肪定量。与2E-VIBE Dixon相比,6E-VIBE Dixon所测肝脏脂肪分数可重复性更高,结果更加准确。Objective To evaluate the feasibility of different MR Dixon techniques for quantification of liver fat in non-alcoholic fatty liver disease(NAFLD)in adults.Methods 54 patients in the case group and 21 patients in the control group were prospectively collected.All subjects underwent 3.0T MR Dixon imaging within 1 week after CT examination of the abdomen.The ratios ofthe liver/spleen CT values,2E-VIBE Dixon(Double Echo Magnetic Resonance Water and Fat Separation Technology)hepatic fat fraction(HFF),6E-VIBE Dixon(6-echo magnetic resonance water-fat separation technology)proton density fat fraction(PDFF)and R 2*were all measured.The consistency and reproducibility of the two observers'measurements,the discrepancy of the liver fat quantification measured on 2E-VIBE Dixon and 6E-VIBE Dixon between and among thedifferentgroups were all statistically analyzed.Results High degree of consistency(ICC,0.946-0.993)and reproducibility(LOA,1.43%~3.72%)were found for liver fat quantification with the techniques of 2E-VIBE Dixon and 6E-VIBE Dixon between the two observers,with the slightly higher performance in 6E-VIBE Dixon.In addition,significant differences were found in HFF and PDFF among the different groups or intra-groupsquantified by the technique of 2E-VIBE Dixon or 6E-VIBE Dixon(P<0.05),with the higher HFF acquired by 2E-VIBE Dixon(P<0.05).Conclusion High consistency and reproducibility could be acquired by the sequences of 2E-VIBE Dixon and 6E-VIBE Dixon in NAFLD.Although HFF or PDFF could be quantitatively assessed on 2E-VIBE Dixon or 6E-VIBE Dixon,higher reproducibility and more accuracy could be detected on 6E-VIBE Dixon.
