摘　　要：目的探讨经颈静脉肝内门体分流术(TIPS)对门静脉高压患者脾脏体积及血小板数量的影响。方法选取并分析101例门静脉高压患者TIPS手术前后脾脏体积及血小板数量变化。结果根据纳入及排除标准,共纳入101例门静脉高压患者,术前脾脏体积(909.95±415.71)cm^(3),术后脾脏体积(791.63±398.97)cm^(3)(P<0.01),TIPS术后脾脏体积缩小77例(76.24%),脾脏体积增大24例(23.76%);术前血小板(67.36±44.64)×10^(9)/L,术后血小板(64.41±47.96)×10^(9)/L(P=0.49),血小板升高39例(38.61%),血小板降低62例(61.39%)。结论门静脉高压患者TIPS术后脾脏体积明显缩小,但血小板数量并未发生明显变化,且两者之间不存在明显的相关性。Objective To investigate the effects of transjugular intrahepatic portal shunt(TIPS)on spleen volume and platelet count in patients with portal hypertension.Methods The changes of spleen volume and blood cell count in patients with portal hypertension before and after TIPS surgery were retrospectively analyzed.Results According to inclusion and exclusion criteria,a total of 101 patients with portal hypertension were included.Preoperative spleen volume was(909.95±415.71)ml,postoperative spleen volume was(791.63±398.97)ml(P<0.01),and spleen volume decreased in 77 patients(76.24%)after TIPS.Spleen volume increased in 24 cases(23.76%).Platelet count was(67.36±44.64)×10^(9)/L before operation and(64.41±47.96)×10^(9)/L after operation(P=0.49).Platelet count increased in 39 cases(38.61%)and decreased in 62 cases(61.39%).Conclusion Spleen volume is significantly reduced in patients with portal hypertension after TIPS,but platelet count doed not change significantly,and there is no obvious correlation between the two.
