摘 要:目的探讨双能CT二代能谱成像(monoenergetic plus,Mono+)技术在肠系膜下动脉(inferior mesenteric artery,IMA)显示中的价值。方法选取行全腹部双能量CT增强检查的患者共146例。通过后处理获得常规120 kVp和40 keV、50 keV、60 keV、70 keV、80 keV、90 keV单能量能级图像,分别对上述7组图像显示IMA的图像质量进行客观定量分析(包括CT值、信噪比、对比噪声比)及主观总体评分(包括图像清晰度和图像锐利度)。采用单因素方差分析和Kruskal-Walls H检验进行统计学分析。结果IMA图像质量客观评估对比噪声比(contrast to noise ratio,CNR):7组图像中40 keV单能级图像最高,差异有统计学意义(P<0.05)。IMA图像质量客观评估信噪比(Signal to noise ratio,SNR):常规120 kVp图像最高,差异有统计学意义(P<0.05)。IMA图像质量主观评估:40 keV单能级图像主观总体评分显著高于其他6组图像,两两比较差异均有统计学意义(P<0.05)。结论双能CTMono+40 keV单能级图像可以明显优化IMA的显示。Objective To investigate the value of dual-energy CT monoenergetic plus(Mono+)in inferior mesenteric artery(IMA).Methods A total of 146 patients who underwent full-abdominal dual-energy CT enhanced examination were retrospectively collected.Conventional 120 kVp and single energy level images of 40 keV,50 keV,60 keV,70 keV,80 keV and 90 keV were obtained by post-processing.Objective quantitative analysis(including CT value,signal-to-noise ratio,contrast to noise ratio)and subjective overall score(including image resolution and image sharpness)were performed on the image quality of IMA in the above 7 groups of images.One-way ANOVA and Kruskal-Walls H test were used for statistical analysis.Results The contrast to noise ratio(CNR)of IMA image quality was objectively evaluated.Among the 7 groups of images,the 40 keV single-level image was the highest,and the difference was statistically significant(P<0.05).IMA image quality was objectively evaluated by Signal to noise ratio(SNR).Conventional 120 kVp image had the highest value,and the difference was statistically significant(P<0.05).Subjective evaluation of IMA image quality was as follows:the overall subjective score of 40 keV single-level images was significantly higher than that of the other 6 groups,and the pair comparison showed statistically significant differences(P<0.05).Conclusion Dual-energy CT Mono+40 keV is the optimal energy level displayed by IMA.
