多模态超声应用于冈上肌腱损伤关节镜术前诊断的效果分析

摘　　要：目的 探究多模态超声应用于冈上肌腱损伤关节镜术前诊断的临床效果。方法 选取2020年1月—2021年11月常熟市第一人民医院收治的100例疑似冈上肌腱损伤且需适时接受肩关节镜手术治疗患者为研究对象,所有疑似患者均需接受多模态超声检查与特殊体征检查,对比多模态超声检查与特殊体征检查的诊断效能。结果 利用多模态超声检查术诊断冈上肌腱损伤的灵敏度、特异度分别为98.44%(63/64)、91.67%(33/36),均高于特殊体征检查(落臂试验、Neer征检查、零度位外展抗阻试验、Hawkins征检查、Jobe试验),差异有统计学意义(χ^(2)=39.237、41.011、41.011、21.955、39.237、16.756、15.126、10.667、8.036、18.463,P<0.001)。结论 多模态超声应用于冈上肌腱损伤关节镜术前诊断的临床效果显著。因此,多模态超声检查术可将广泛用于进行关节镜术前诊断的冈上肌腱损伤患者,促使诊断质量、诊断价值显著提升。Objective To explore the clinical effect of multimodal ultrasound in the preoperative arthroscopic diagnosis of supraspinatus tendon injury.Methods 100 patients with suspected supraspinatus tendon injury and in need of timely shoulder arthroscopic surgery admitted to Changshu First People's Hospital from January 2020 to November 2021 were selected as the research objects.All suspected patients were subjected to multimodal ultrasound examination and special signs examination,and the diagnostic efficacy of multimodal ultrasound examination and special signs examination was compared.Results The sensitivity and specificity of multimodal ultrasonography in the diagnosis of supraspinatus tendon injury were 98.44%(63/64)and 91.67%(33/36),respectively,which were higher than the special signs(falling arm test,Neer sign test,zero position abduction resistance test,Hawkins sign test,Jobe test),the difference was statistically significant(χ^(2)=39.237,41.011,41.011,21.955,39.237,16.756,15.126,10.667,8.036,18.463,P<0.001).Conclusion The application of multimodal ultrasound in the preoperative arthroscopic diagnosis of supraspinatus tendon injury has significant clinical effect.Therefore,multimodal ultrasonography can be widely used in the preoperative diagnosis of patients with supraspinatus tendon injury under arthroscopy,which significantly improves the quality and value of diagnosis.
