摘 要:目的 探讨肺部超声联合心脏超声对小儿急性呼吸窘迫综合征的诊断价值。方法 选取2019年2月—2021年9月河西学院附属张掖人民医院治疗的100例疑似患有小儿急性呼吸窘迫综合征的患儿为研究对象,所有患儿均接受肺部超声、心脏超声检查,此次研究的金标准为儿童急性肺损伤委员会2015年定制的诊断标准,分析对比检查结果。结果 金标准阳性为60例;肺部超声检查、心脏超声检查、联合检查的阳性检出率分别为58.00%、59.00%、60.00%;联合检查灵敏度98.33%、特异度97.50%、阳性预测值98.33%、阴性预测值97.50%、准确性98.00%均高于单一肺部超声检查、心脏超声检查诊断效能,差异有统计学意义(P<0.05)。结论 使用肺部超声联合心脏超声的方式诊断小儿急性呼吸窘迫综合征,具有较高的诊断效能,检出率更高。Objective To explore the diagnostic value of pulmonary ultrasound and cardiac ultrasound in pediatric acute respiratory distress syndrome.Methods 100 children suspected of suffering from acute respiratory distress syndrome in children treated by Zhangye People's Hospital affiliated to Hexi University from February 2019 to September 2021 were selected as the study subjects.All children received lung ultrasound and cardiac ultrasound examination.The gold standard of this study was the diagnostic standard customized by the Children's Acute Lung Injury Committee in 2015,and the results were analyzed and compared.Results The gold standard was 60 cases;the positive detection rates of lung ultrasound,cardiac ultrasound and combined examination were 58.00%,59.00%and 60.00%respectively;the sensitivity,specificity,positive predictive value,negative predictive value and accuracy of combined examination were 98.33%,97.50%,97.50%and 98.00%,respectively,higher than those of single lung ultrasound and cardiac ultrasound,the difference was statistically significant(P<0.05).Conclusion The use of pulmonary ultrasound combined with cardiac ultrasound in the diagnosis of acute respiratory distress syndrome in children has higher diagnostic efficiency and higher detection rate.
