摘  要:目的 探讨CT与核磁共振在早期脑梗死中的诊断效果。方法 回顾性分析2020年7月—2021年7月长春市人民医院神经内科接诊的80例早期脑梗死患者临床资料,所有患者均行CT与核磁共振检查,对比两种诊断结果。结果 经CT检查后发现病灶于额叶7例(53.84%)、脑干9例(56.25%)、基底节14例(63.63%),经核磁共振检查后发现病灶于额叶12例(92.30%)、脑干15例(93.75%)、基底节21例(95.45%),核磁共振检出率明显高于CT,差异有统计学意义(χ^(2)=4.513、4.167、5.029,P<0.05);经CT检查后发现病灶于枕叶7例(63.63%),颞叶9例(69.23%)、小脑4例(80.00%),经核磁共振检查后发现病灶于枕叶9例(81.81%),颞叶11例(84.61%)、小脑5例(100.00%),两者检出率比较,差异无统计学意义(P>0.05)。CT对于病灶≤5 mm小病灶的检出率明显低于核磁共振,差异有统计学意义(χ^(2)=15.121,P<0.05)。核磁共振的影像整体质量评分和伪影情况评分均高于CT,差异有统计学意义(t=2.523、3.148,P<0.05)。结论 核磁共振在早期脑梗死的诊断中具有更好的诊断效果,其能够更好的发现早期脑梗死的微小病灶,更利于临床的诊断和治疗。Objective To investigate the diagnostic effect of CT and nuclear magnetic resonance in early cerebral infarction.Methods The clinical data of 80 patients with early cerebral infarction admitted by the Department of Neurology of Changchun People's Hospital from July 2020 to July 2021 were retrospectively analyzed.All patients underwent CT and MRI examinations,and the two diagnostic results were compared.Results After CT examination,the lesions were found in the frontal lobe in 7 cases(53.84%),brain stem in 9 cases(56.25%),and basal ganglia in 14 cases(63.63%).After MRI examination,the lesions were found in the frontal lobe in 12 cases(92.30%),brain stem in 15 cases(93.75%),and basal ganglia in 21 cases(95.45%).The detection rate of MRI was significantly higher than that of theCT,and the difference was statistically significant(χ^(2)=4.513,4.167,5.029,P<0.05);after CT examination,the lesions were found in occipital lobe in 7 cases(63.63%),temporal lobe in 9 cases(69.23%)and cerebellum in 4 cases(80.00%),after MRI examination,the lesions were found in occipital lobe in 9 cases(81.81%),temporal lobe in 11 cases(84.61%)and cerebellum in 5 cases(100.00%),there was no statistically significant difference between the two detection rates(P>0.05).The detection rate of CT for small lesions≤5 mm was significantly lower than that of MRI(χ^(2)=15.121,P<0.05).The overall image quality score and artifact score of MRI were higher than those of CT,and the difference was statistically significant(t=2.523,3.148,P<0.05).Conclusion Nuclear magnetic resonance has better diagnostic effect in the diagnosis of early cerebral infarction,and its ability to better detect the microscopic lesions of early cerebral infarction is more conducive to clinical diagnosis and treatment.
