丹参化瘀汤联合芙蓉膏治疗肉芽肿性乳腺炎并发下肢结节红斑临床观察

摘　　要：目的:通过口服丹参化瘀汤联合芙蓉膏外敷治疗肉芽肿性乳腺炎(GM)并发下肢结节性红斑(EN),分析控制炎症表现的临床相关指标,探讨该法对GM并发下肢EN的疗效。方法:收集在安徽中医药大学第一附属医院乳腺外科就诊,符合GM并发下肢EN的40例患者,按随机数字表法随机分成两组,每组20例。研究组予丹参化瘀汤口服,对照组予头孢地尼胶囊口服,两组患者同时采用院内制剂芙蓉膏外敷下肢结节性红斑处,观察周期为14 d。观察且记录患者治疗前后红斑数目、颜色、平均直径大小、VAS疼痛评分,比较治疗前后外周血白细胞计数(WBC)、中性粒细胞百分比(NE)、超敏C反应蛋白(hsCRP)、血沉(ESR)相关实验室指标水平变化,评估两组患者疗效。结果:治疗后研究组患者下肢红斑数目、平均直径大小及疼痛评分与对照组患者比较,差异有显著统计学意义(P<0.01),且研究组低于对照组;治疗后研究组患者外周血WBC、NEUT、hsCRP、ESR水平低于对照组,差异有统计学意义(P<0.05)。结论:丹参化瘀汤口服联合芙蓉膏外敷治疗GM并发下肢EN时,其控制临床炎症的各项指标均优于对照组,表明该法治疗GM并发下肢EN,可以明显缓解炎症反应,从而改善临床症状,值得临床推广应用。Objective:To analyze the clinical indicators related to the control of inflammatory manifestations through oral Danshen Huayu Decoction combined with Furong Ointment in the treatment of granulomatous mastitis(GM)com-plicated with erythema nodosum(EN)of lower extremities,and to explore the efficacy of this treatment.Methods:40 patients with GM complicated with EN of lower extremities in the Department of Breast Surgery of The First Affiliated Hospital of Anhui University of Traditional Chinese Medicine were collected and randomly divided into 2groups ac-cording to random number table method,with 20patients in each group.The study group was given Danhua Decoction orally,and the control group was given cefdinir orally.Both groups were treated with Furong Ointment prepared in the first Affiliated Hospital of Anhui University of Traditional Chinese Medicine for external application of erythema nodo-sum of lower limbs.The observation period was 14days.The amount,color,and average diameter of those patients'no-dosums,as well as the VAS pain score,were observed and recorded throughout the treatment,also the level changes of the relevant laboratory indexes,such as peripheral white blood cell count(WBC),neutrophil percentage(NE),hyper-sensitive c-reactive protein(hsCRP),blood sedimentation(ESR),were compared in order to assess the treatment effi-cacy of two groups.Results:After treatment,the amount of erythema of lower limbs,average diameter,and pain score of patients in the study group were significantly different from those in the control group(P<0.01),and the study group was lower than the control group.After the treatment,the levels of WBC,NEUT,hsCRP,and ESR in the periph-eral blood of patients in the study group were lower than those in the control group,and the differences were statistical-ly significant(P<0.05).Conclusion:When Danshen Huayu Decoction was taken orally and Furong Ointment was ap-plied externally in the treatment of GM complicated with EN in lower extremities,all the indexes of clinical inflamma-tion c
