摘　　要：外伤性视神经萎缩是由于外伤导致视神经病变,视网膜神经节细胞逐渐变性坏死,引起视力降低甚至失明的疾病。左韬教授认为,玄府与瞳神是该病关系最密切的结构,其病机可分为两个阶段,即初期玄府瘀闭,病久瞳神失养。初期撞击之后,直伤瞳神,玄府瘀闭,血瘀气滞,肝之真血不能上行入眼,瞳神失于濡养而目不能视物,可归为气滞血瘀型;病久可因情志不畅、肝肾亏虚,导致瞳神日益萎靡,可分为肝气郁结型、肝肾不足型。治疗当以中药加针刺,辨证用药取穴,初期以宣通玄府为主,活血化瘀,后期以滋养瞳神为主,同时肝气郁结型以疏肝解郁为主,肝肾不足型则补益肝肾,可奏全功。Traumatic optic atrophy is caused by optic neuropathy caused by trauma,and retinal ganglion cells gradually degenerate and necrotic,resulting in reduced vision and even blindness.Professor Zuo Tao thinks,the pathogenesis of atrophy of traumatic optic nerve can be divided into two stages,namely initial xuanfu yu is closed,disease long pupil god is lost to keep.After the initial impact,pupil god was directly injured,the Xuanfu stasis,blood stasis and Qi stag-nation,the true blood of liver could not reach the eye,pupil god was lost and the eyes could not see,which could be classified as Qi stagnation and blood stasis type;Disease for a long time due to poor mood,liver and kidney deficiency,resulting in pupils increasingly disoriented,can be divided into liver Qi stagnation type,liver and kidney insufficiency type.When the treatment with traditional Chinese medicine and acupuncture,syndrome differentiation medicine acu-point selection,early to xuantong xuanfu based,invigorate blood stasis,later to nourish the eyes god based,at the same time liver Qi stagnation type to dredging liver and relieving depression,liver and kidney deficiency type tonifying liver and kidney,can play full effect.
