摘 要:目的 探讨对儿童Ⅰ型糖尿病(T1DM)采用持续胰岛素泵输注的临床干预效果。方法 本次纳入对象为郑州大学第一附属医院儿科2021年6月—2022年6月收治的T1DM患儿74例,按治疗方法不同将实施每天多次胰岛素注射治疗(MDI)的34例作为对照组,将实施持续胰岛素泵输注(CSII)的40例作为观察组,比较两组临床干预效果。结果 两组治疗前血糖水平比较差异均无统计学意义(P>0.05),而治疗后两组空腹血糖(FPG)、餐后2 h血糖(2 h PG)及糖化血红蛋白(HbA1c)均明显低于同组治疗前且观察组明显低于对照组(P<0.05);两组治疗前CGMS结果比较差异无统计学意义(P>0.05),治疗后两组各项指标均明显低于对照组且观察组明显低于对照组(P<0.05);观察组血糖达标时间与胰岛素用量比对照组明显减少(P<0.05)。结论 CSII治疗T1DM可快速有效控制血糖,且安全性高,值得推广。Objective To investigate the clinical intervention effect of continuous insulin pump infusion in children with type Ⅰ diabetes mellitus(T1DM). Methods Totally 74 children with T1DM admitted to our hospital from June 2021 to June 2022 were enrolled in this study. According to different treatment methods, 34 children were treated with multiple daily insulin injection therapy(MDI) as the control group, and 40 children were treated with continuous insulin pump infusion(CSII) as the observation group. The clinical intervention effects of the two groups were compared. Results There was no significant difference in blood glucose levels between the two groups before treatment(P>0.05), but after treatment, FPG, 2 h PG and HbA1c in the two groups were significantly lower than those before treatment, and the observation group was significantly lower than the control group(P<0.05). There was no significant difference in the results of CGMS between the two groups before treatment(P>0.05). After treatment, the indicators of the two groups were significantly lower than those of the control group, and the observation group was significantly lower than that of the control group(P<0.05). The time to reach the standard of blood glucose and the amount of insulin in the observation group were shorter and less than those in the control group(P<0.05). Conclusion CSII can control blood glucose quickly and effectively in the treatment of T1DM with high safety, which is worthy of promotion.
