对比骨质疏松性胸腰椎压缩骨折应用PVP与PKP治疗的临床效果

摘　　要：探讨经皮椎体成形术(PVP)与经皮椎体后凸成形术(PKP)治疗骨质疏松性胸腰椎体压缩骨折的临床效果及对功能恢复的影响。方法:选取我院骨科2017年3月-2020年2月收治的200例骨质疏松性胸腰椎压缩骨折患者,作为本次的研究对象。按照数字表法随机分为2组,每组各100例。分别采用经皮椎成形术治疗(PVP组)、经皮椎后凸成形术治疗(PKP组)。对2组手术指标、术后并发症发生率、疼痛改善情况、腰椎功能恢复情况进行对比分析。结果:PVP组手术时间、X线曝光次数均明显低于PKP组(P<0.05);2组骨水泥用量无明显差异(P>0.05);PKP组骨水泥渗漏发生率低于PVP组(P<0.05);术后2组腰椎功能恢复情况、疼痛改善情况差异无统计学意义(P>0.05)。结论:对胸腰椎骨质疏松性椎体压缩骨折患者,应用PVP与PKP治疗,均可以起到改善患者的骨折疼痛感受,促进患者功能恢复的作用。临床效果较好,具有较高的应用价值。使用PKP治疗能降低术中骨水泥渗漏的发生率,而应用PVP治疗能够减少术中患者X线曝光次数,手术时间更短。Objective:To investigate the clinical effect and effect of percutaneous vertebroplasty(PVP)and per-cutaneous kyphoplasty(PKP)in the treatment of osteoporotic thoracolumbar compression fracture and their effects on functional recovery.Methods:A total of 200 patients with osteoporotic thoracolumbar compression fracture admitted to the Department of Orthopedics of our hospital from March 2017 to February 2020 were selected as the research ob-jects.According to the number table method,they were randomly divided into 2 groups,each group with 100 cases.Percutaneous vertebroplasty and percutaneous kyphoplasty were used.The surgical index,postoperative complication rate,pain improvement and lumbar function recovery of the two groups were compared and analyzed.Results:The operation time and X-ray exposure times in the PVP group were significantly lower than those in the PKP group(P<0.05),there was no significant difference in the amount of bone cement in the two groups(P>0.05),;P the in-cidence of bone cement leakage in the KP group was lower than that in the PVP group(P<0.05),and there was no significant difference in the recovery of lumbar spine function and pain improvement between the two groups after sur-gery(P>0.05).Conclusion:For patients with thoracolumbar osteoporotic vertebral compression fractures,PVP and PKP treatment can improve the fracture pain and promote the functional recovery of patients.The clinical effect is good and has high application value.The use of PKP treatment can reduce the incidence of intraoperative cement leakage,while the use of PVP treatment can reduce the number of X-ray exposures and shorten the operation time of intraoperative patients.
