摘 要:目的探讨血氧灌注指数对新生儿休克患儿脏器功能损害程度的预测价值。方法选取2020年2月—2021年2月嘉兴市妇幼保健院收治的50例新生儿休克患儿为观察组,另选取同期于在医院进行健康体检的健康儿童40例为对照组。检测血氧灌注指数水平,分析血氧灌注指数在新生儿休克脏器功能损害患儿中的表达及对脏器功能损害程度的预测价值。结果对照组40例新生儿出生后1~7 d血氧灌注指数水平分别为1.89±0.23、1.95±0.25、2.06±0.37、2.26±0.38、2.35±0.40、2.56±0.45、2.78±0.52,观察组50例患儿血氧灌注指数水平为0.75±0.15、0.80±0.22、0.92±0.25、0.93±0.26、0.98±0.31、1.01±0.32、1.21±0.36,观察组50例患儿血氧灌注指数水平低于对照组40例新生儿(t=28.330、23.190、17.390、19.220、18.310、19.070、16.890,均P<0.05)。观察组20例轻度休克患儿血氧灌注指数水平为1.12±0.33,19例中度休克患儿血氧灌注指数水平为0.90±0.28,11例重度休克患儿血氧灌注指数水平为0.65±0.12,与轻度休克患儿相比,中度、重度休克患儿血氧灌注指数水平较低,差异有统计学意义(t=2.239/4.532,P<0.05);与中度休克患儿相比,重度休克患儿血氧灌注指数水平较低(t=2.800,P<0.05)。与1个脏器功能损害休克患儿相比,2-3、>3个脏器功能损害休克患儿血氧灌注指数水平较低(1.00±0.31 vs.0.94±0.27 vs.0.68±0.11,P<0.05);与2~3个脏器功能损害休克患儿相比,>3个脏器功能损害休克患儿血氧灌注指数水平较低(0.94±0.27 vs.0.68±0.11,P<0.05)。Ⅰ型呼吸衰竭、Ⅱ型呼吸衰竭、脑功能衰竭、肺出血、胃肠道功能衰竭及肾功能衰竭休克患儿血氧灌注指数水平比较(P>0.05)。血氧灌注指数与观察组不同病情严重程度呈负相关(P<0.05)。logistic回归分析显示血氧灌注指数水平降低是脏器功能损害加重的危险因素。结论血氧灌注指数在新生儿休克脏器功能损害患儿中水平较低,�Objective,To analyze the value of blood oxygen perfusion index in predicting the degree of organ damage in children with neonatal shock.Methods A total of 50 children with neonatal shock admitted to our hospital from February 2020 to February 2021 were selected as the research group,and 40 healthy children who underwent physical examination in our hospital during the same period were selected as the control group.The level of blood oxygen perfusion index was detected,and the expression of blood oxygen perfusion index in children with neonatal shock with organ dysfunction and its predictive value for the degree of organ dysfunction were analyzed.Results The blood oxygen perfusion index levels of 40 newborns in the control group were 1.89±0.23,1.95±0.25,2.06±0.37,2.26±0.38,2.35±0.40,2.56±0.45,2.78±0.52,respectively.The blood oxygen perfusion index levels of 50 newborns in the study group were 0.75±0.15,0.80±0.22,0.92±0.25,0.93±0.26,0.98±0.31,1.01±0.32,1.21±0.36,The blood oxygen perfusion index of 50 children in the study group was lower than that of 40 newborns in the control group(t=28.330,23.190,17.390,19.220,18.310,19.070,16.890,all P<0.05).In the study group,the blood oxygen perfusion index of 20 children with mild shock was 1.12±0.33,the blood oxygen perfusion index of 19 children with moderate shock was 0.90+0.28,and the blood oxygen perfusion index of 11 children with severe shock was 0.65±0.12.Compared with children with mild shock,the blood oxygen perfusion index of children with moderate and severe shock was lower,with statistical difference(t=2.239,4.532,all P<0.05);Compared with children with moderate shock,children with severe shock had lower blood oxygen perfusion index(t=2.800,all P<0.05).The blood oxygen perfusion index of children with 2-3 and>3 organ function impairment shock was lower than that of children with 1 organ function impairment shock(1.00±0.31 us.0.94±0.27 vs.0.68±0.11,P<0.05);Compared with children with 2-3 organ function impairment shock,the blood oxygen perfusion
