杞菊地黄丸加味联合西医疗法对早期糖尿病肾病肝肾阴虚夹瘀证的临床疗效

摘　　要：目的探讨杞菊地黄丸加味联合西医疗法治疗早期糖尿病肾病(diabetic kidneydisease,DKD)肝肾阴虚夹瘀证的临床效果。方法选取2018年3月—2020年3月在齐齐哈尔市第一医院确诊的80例DKD患者为研究对象,随机分为西医组(40例,常规降糖、降压、降脂)和中西医联合组(40例,对照组基础上联合杞菊地黄丸加味)。对比两组治疗效果。结果治疗2个月后,西医组肝肾阴虚夹瘀证症候积分显著高于中西医联合组,临床有效率显著低于中西医联合组,差异有统计学意义(P<0.05)。治疗前两组患者血清血尿素氮(BUN)、血肌酐(Scr)、胰岛素样生长因子-1(IGF-1)、白介素-8(IL-8)、转化生长因子-β1(TGF-β1),尿蛋白排泄率(UAER)比较,差异无统计学意义(P>0.05);治疗后,两组各指标水平均有所下降,且中西医联合组水平均低于西医组,差异有统计学意义(P<0.05)。结论杞菊地黄丸加味联合西医疗法治疗能明显改善早期糖尿病肾病肝肾阴虚夹瘀证患者的中医证候和肾功能,对IGF-1、TGF-β1、IL-8水平的影响可能是其疗效途径之一。Objective To investigate the clinical effect of Qiju Dihuang Pills Modified combined with radix antipathy and Western medicine on the syndrome of liver and kidney Yin deficiency with blood stasis in early diabetic nephropathy.Methods A total of 80 DKD patients diagnosed in the First Hospital of Qiqihar from March 2018 to March 2020 were selected as research objects,and randomly divided into western medicine group(40 cases,routine hypoglycemic,hypertensive,lipid lowering)and combined traditional Chinese and western medicine group(40 cases,the control group was combined with Qiju Dihuang Pills Modified).The therapeutic effect of the two groups was compared.Results After 2 months of treatment,the score of syndrome of liver-kidney Yin deficiency with blood stasis in the western medicine group was significantly higher than that in the combined traditional Chinese and western medicine group,and the clinical effective rate was significantly lower than that in the combined traditional Chinese and western medicine group,the difference was statistically significant(P<0.05).Compare the serum urea nitrogen(BUN),serum creatinine(Scr),insulin-like growth factor-1(IGF-1),interleukin-8(IL-8),transforming growth factor-β1(TGF-β1),urinary protein excretion rate(UAER)of the two groups before treatment,the difference was not statistically significant(P>0.05);after treatment,all indexes of the two groups were decreased,the average level in the combined tradi‐tional Chinese and western medicine group was lower than that in the western medicine group,with a statistically significant difference(P<0.05).Conclusion Qiju Dihuang Pills Modified combined with Western medicine therapy can significantly improve the TCM syndrome and renal function in patients with early diabetic nephropathy with syndrome of liver and kidney Yin deficiency and blood stasis.The influence on IGF-1,TGF-β1 and IL-8 levels may be one of the therapeutic approaches.
