放射式冲击波联合威伐光对桡骨茎突狭窄性腱鞘炎的治疗效果

摘　　要：目的:探究放射式冲击波联合威伐光对桡骨茎突狭窄性腱鞘炎的治疗效果。方法:选取本院2020年1月至2022年1月份就诊的80例桡骨茎突狭窄性腱鞘炎患者,依照随机数字表法分为对照组、研究组(n=40),对照组患者应用威伐光进行3次•w^(-1),每次20 min的照射治疗,研究组在对照组照射治疗的基础上加用放射式冲击波进行5 d•次^(-1)的冲击波治疗。比较两组治疗效果,在治疗前及治疗4 w后应用疼痛视觉模拟评分(Visual analogue pain scales,VAS)、腕关节活动度(Range of motion,ROM)评估患者疼痛程度及腕关节活动度,以Quinell病变程度评分、腕关节功能Cooney评分评估患者病变程度及腕关节功能,采用酶联免疫吸附试验检测肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、C-反应蛋白(C-reactive protein,CRP)水平,通过超声波诊断仪检测血流动力学参数收缩期峰值(Peak systolic velocity,PSV)、舒张末期峰值(Enddiastolic peak,DEV)、搏动指数(Pulsation index,PI)、阻力指数(Resistance index,RI)变化。结果:研究组总有效率高于对照组(P<0.05);与治疗前相比,各组患者疼痛评分、腕关节活动度、Cooney评分、Quinell评分均得到明显改善(P<0.05),其中研究组更为显著(P<0.05);与治疗前相比,各组TNF-α、CRP水平、均明显降低(P<0.05),其中研究组更为显著(P<0.05);与治疗前相比,各组PSV、EDV、PI、RI水平均明显升高(P<0.05),其中研究组更为显著(P<0.05)。结论:放射式冲击波联合威伐光可改善桡骨茎突狭窄性腱鞘炎患者临床症状,利于治疗效果提升,缓解疼痛,促进关节功能恢复。Objective:To explore the therapeutic effect of radiation shock waves combined with water-filtered infraredA on radial styloid stenosing tenosynovitis.Methods:A total of 80 patients with radial styloid tenosynovitis treated in our hospital from January 2020 to January 2022 were selected and divided into a control group and a study group according to the random number table method(n=40).Patients in the control group were treated 3 times•w^(-1)irradiation with water-filtered infrared-A,20 min each time.Based on radiation therapy in the control group,the study group was treated with radiation shock waves for every five days.The treatment effect of the two groups was compared,visual analog pain scales(VAS)and wrist range of motion(ROM)were used to evaluate the pain and wrist range of motion before and after 4 weeks of treatment,the levels of tumor necrosis factor-α(TNF-α)and C-reactive protein(CRP)were detected by enzyme-linked immunosorbent assay,hemodynamic parameters including peak systolic velocity(PSV),end-diastolic peak(DEV),pulsation index(PI),and resistance index(RI)were detected by the ultrasonic diagnostic instrument.Results:The total effective rate of the study group was higher than that of the control group(P<0.05);compared with pretreatment,pain score,ROM,Cooney score,and Quinell score were significantly improved in all groups(P<0.05),the study group was more significant(P<0.05).Compared with pretreatment,the levels of TNF-αand CRP in all groups were significantly decreased(P<0.05),and the study group was more significant(P<0.05).Compared with pretreatment,the levels of PSV,EDV,PI,and RI in each group were significantly increased(P<0.05),and the study group was more significant(P<0.05).Conclusion:Injection shock wave combined with waterfiltered infrared-A can improve the clinical symptoms of patients with radial styloid stenotic tenosynovitis,which is beneficial to improve the treatment effect,relieve pain,and promote the recovery of joint function.
