肾益康胶囊辅助甲泼尼松、他克莫司治疗对特发性膜性肾病患者疗效、肾功能、炎性因子、血栓弹力图的影响

摘　　要：目的:研究益康胶囊辅助甲泼尼松、他克莫司治疗特发性膜性肾病(Idiopathic membranous nephropathy,IMN)患者的疗效。方法:收集我院2018年7月到2020年7月收治的82例IMN患者,根据治疗药物不同,分为常规药物组和肾益康胶囊组(n=41)。常规药物组予以口服糖皮质激素甲泼尼松和免疫抑制剂他克莫司,肾益康胶囊组在常规药物组基础上口服肾益康胶囊辅助治疗。治疗6 m后,评估两组总有效率;治疗前和治疗6 m后,采取免疫透射比浊法检测患者血清胱抑素C和血肌酐水平;尿酶动力法检测血清尿素氮水平;乳胶增强免疫比浊法检测24 h的尿蛋白定量等肾功能相关指标;采取酶联免疫吸附法检测患者血清肿瘤坏死因子α(Tumor necrosis factorα,TNF-α)、转化生长因子β(Transforming growth factorβ,TGF-β)、白细胞介素8(Interleukin-8,IL-8)、IL-31水平;采取全自动血栓弹力图分析仪检测患者的反应时间、凝血时间、凝固角、凝血指数等血栓弹力图相关指标。结果:治疗6 m后,肾益康胶囊组总有效率高于常规药物组(P<0.05);肾益康胶囊组血清血肌酐、24 h尿蛋白定量、血肌酐、尿素氮、TNF-α、TGF-β、IL-8、IL31、凝固角、凝血指数低于常规药物组,反应时间、凝血时间高于常规药物组(P<0.05);肾益康胶囊组不良反应总发生率低于常规药物组(P<0.05)。结论:肾益康胶囊辅助甲泼尼松、他克莫司治疗IMN的疗效显著,能改善机体的肾功能,减轻机体的炎性反应状态,缓解机体的高凝状态,减少不良反应的发生。Objective:To study the adjuvant therapeutic effect of the Shenyikang capsule on methylprednisone and tacrolimus in the treatment of idiopathic membranous nephropathy(IMN).Methods:Eighty-two cases of IMN patients admitted to our hospital from July 2018 to July 2020 were collected and divided into routine drug group and Shenyikang capsule group according to the different drugs(n=41).The routine drug group was treated with oral methylprednisone and tacrolimus,and the Shenyikang capsule group was treated with oral Shenyikang capsule based on methylprednisone and tacrolimus.After 6 months of treatment,the total effective rate was evaluated;before and after 6 months of treatment,immunoturbidimetry was used to measure the serum levels of cystatin C and serum creatinine;urine enzyme kinetic method was used to detect serum level of urea nitrogen;latex enhanced immunoturbidimetry was used to detect the 24 h urine protein content;enzyme-linked immunosorbent assay was used to detect the serum levels of tumor necrosis factorα(TNF-α),transforming growth factorβ(TGF-β),interleukin 8(IL-8)and IL-31;the automatic thromboelastogram analyzer was used to detect the reaction time,coagulation time,coagulation angle,and coagulation index.Results:After 6 months of treatment,the total effective rate of the Shenyikang capsule group was higher than that of the conventional drug group(P<0.05);serum creatinine,24 h urine protein content,serum creatinine,urea nitrogen,TNF-α,TGF-β,IL-8,IL-31,coagulation angle,and coagulation index in Shenyikang capsule group were lower than those in the routine drug group,while reaction time and coagulation time were higher than those in the routine drug group(P<0.05).The total incidence of adverse reactions in the Shenyikang capsule group was lower than that in the routine drug group(P<0.05).Conclusion:Shenyikang capsule can enhance the effect of methylprednisone and tacrolimus in the treatment of IMN by improving the renal function of the body,alleviating the inflammatory reaction state and the hyperco
