摘 要:目的 分析探究影响高危儿全身运动质量的危险因素分析,进而进行早期干预,促进儿童的神经发育。方法 选择2020年1—12月本院新生儿室住院治疗的高危儿88例作为研究对象,所有患儿均完成了GMs扭动阶段和不安运动阶段评估,最终采用非参数检验Mann-Whitney U检验、单因素分析、二元Logistic回归分析确定影响GMs质量的危险因素。结果 Logistic回归分析显示:扭动阶段有统计学意义的危险因素是新生儿缺氧缺血性脑病(Hypoxic ischemic encephalopathy, HIE)(OR=5.735,95%CI:1.032-31.855)、颅内出血(OR=8.278,95%CI:1.853-36.982)、新生儿低血糖症(OR=5.265,95%CI:1.219-22.746),影响扭动阶段异常程度的有统计学意义的危险因素是HIE(OR=15.566,95%CI:1.349-179.59),不安运动阶段有统计学意义的危险因素是新生儿窒息(OR=4.289,95%CI:1.114-16.519)。结论 HIE、颅内出血、新生儿低血糖症、新生儿窒息是影响高危儿全身运动质量的危险因素,对存在上述危险因素的高危儿进行早期识别和早期干预。Objective To analyze the risk factors influencing the quality of general movements(GMs)in high-risk infants,in order to be early intervention and improve neuro-development outcomes.Methods A total of 88 high-risk infants who were receiving hospitalized treatments in neonatal ward of the hospital during January and December 2020 were selected as study subjects.All the infants received the general movements assessment in the stage of writhing movements and fidgety movements.The risk factors for the quality of GMs in high-risk infants were analyzed by Mann-Whitney U test,single factor analysis and binary logistic analysis.Results Binary logistic analysis revealed that the factors significantly associated with the quality of GMs in the stage of writhing movements were hypoxic ischemic encephalopathy(HIE)(OR=5.735,95%CI:1.032-31.855),intracranial bleeding(OR=8.278,95%CI:1.853-36.982),neonatal hypoglycemia(OR=5.265,95%CI:1.219-22.746).The factors significantly associated with the abnormal quality degree of GMs in the stage of writhing movements were HIE(OR=15.566,95%CI:1.349-179.59).The analysis also indicated asphyxia of the newborn was influencing factor for low quality of GMs in the stage of fidgety movements(OR=4.289,95%CI:1.114-16.519).Conclusions HIE,intracranial bleeding,neonatal hypoglycemia,asphyxia of the newborn are risk factors for abnormal GMs results in high-risk infants.Early recognition of these high-risk infants and taking timely intervention can improve neuro-developmental outcomes.
