五味消毒饮联合针灸冲任二脉诸穴治疗急性盆腔炎临床观察

摘　　要：目的:观察五味消毒饮联合针灸冲任二脉诸穴治疗急性盆腔炎的临床疗效。方法:采用随机数字表法将收治的69例急性盆腔炎患者,随机分为对照组(34例)和观察组(35例)。对照组给予常规治疗,观察组加服五味消毒饮并取冲任二脉诸穴针刺。两组共治疗14天。测定两组治疗后血清C反应蛋白、D-二聚体、血清肿瘤坏死因子α、白细胞介素2及白细胞介素6水平;观察两组盆腔超声检查结果、腹痛消失时间、体温和白细胞计数及中性粒细胞比例恢复时间,比较症状积分、总有效率及不良反应发生率。结果:观察组血清C反应蛋白、D-二聚体、血清肿瘤坏死因子α、白细胞介素6水平均低于对照组(P<0.05),白细胞介素2水平高于对照组(P<0.05)。观察组子宫体积、盆腔积液深度及盆腔炎性包块直径均小于对照组(P<0.05)。观察组腹痛消失时间及体温、白细胞计数及中性粒细胞比例恢复时间短于对照组(P<0.05)。不良反应发生率观察组[25.71%(9/35)]与对照组[35.29%(12/34)]比较差异无统计学意义(P>0.05)。总有效率观察组[94.28%(33/35)]高于对照组[82.35%(28/34)](P<0.05)。结论:五味消毒饮联合针灸冲任二脉诸穴治疗急性盆腔炎,可显著提高临床疗效,抑制炎症反应,增强西药的抗炎作用,且安全性好。Objective:To observe clinical effects of Wuwei Xiaodu drink jointed with acupoint moxibustion of Chong and Ren meridians in the treatment of acute pelvic inflammation(API).Methods:All 69 API patients were allocated to 34 cases of the control group and 35 cases of the observation group using random number table method.The control group accepted conventional therapy,and the observation group took Wuwei Xiaodu drink and acupoint moxibustion of Chong and Ren meridians,for 14 days.To determine the levels of CRP,D dimer,TNF-α,IL-2 and IL-6 after the treatment;to observe pelvic ultrasound examination results,disappearing time of abdominal pain,body temperature,WBC counting and the recovery time of neutrophil ratio between both groups,to compare symptom scores,total effective rate and the incidences of adverse reaction.Results:The observation group was lower than the control group in the levels of CRP,D dimer,TNF-αand IL-6(P<0.05),the level of IL-2 higher than that of the control group(P<0.05).Uterine volume,the depth of pelvic effusion and the diameter of pelvic inflammation block of the observation group were less than these of the control group(P<0.05).The observation group was shorter than the control group in disappearing time of abdominal pain,body temperature,WBC counting and the recovery time of neutrophil ratio(P<0.05).The incidences of adverse reaction of the observation group and the control group were[25.71%(9/35)]and[35.29%(12/34)],and the difference had no statistical meaning(P>0.05).Total effective rate of the observation group was[94.28%(33/35)],higher than[82.35%(28/34)]of the control group(P<0.05).Conclusion:Wuwei Xiaodu drink jointed with acupoint moxibustion of Chong and Ren meridians in the treatment of API could remarkably raise clinical effects,inhibit inflammatory reaction and enhance anti-inflammatory effects of Western medicine with good safety.
