中药熏洗在小儿肛瘘切开挂线术后的应用

摘　　要：目的观察中药熏洗用于小儿肛瘘切开挂线术后的临床疗效。方法选取2018年9月至2020年9月浙江中医药大学附属温州中医院收治的肛瘘患儿60例,采用随机数字表法将其分为对照组和观察组,各30例。对照组采取切开挂线术,观察组在对照组的基础上结合中药熏洗。比较两组术前及术后3、7 d的疼痛瘙痒评分、入睡时间及水肿、渗液评分。术后2个月比较两组疗效。结果两组术后3 d的疼痛瘙痒评分及水肿、渗液程度评分低于术前,入睡时间短于术前,术后7 d疼痛瘙痒评分及水肿、渗液程度评分低于术后3 d,入睡时间短于术后3 d,差异有统计学意义(P<0.05)。观察组术后7 d的疼痛瘙痒评分及水肿、渗液程度评分低于同期对照组,入睡时间短于同期对照组,差异有统计学意义(P<0.05)。术后2个月,观察组疗效优于对照组,差异有统计学意义(P<0.05)。结论中药熏洗对小儿肛瘘具有积极的治疗效果,能提高切开挂线术疗效,减轻疼痛,缩短入睡时间及减轻术后水肿、渗液的程度,值得临床进一步推广。Objective To observe the curative effect of steaming and douche with traditional Chinese medicine after therapy of cutting with thread ligation of anal fistula in children.Methods Sixty children with anal fistula admitted to Wenzhou Hospital of Traditional Chinese Medicine Affiliated to Zhejiang Chinese Medical University from September 2018 to September 2020 were selected and divided into observation group and control group by random number table method,with 30 cases in each group.The control group was treated with therapy of cutting with thread ligation,and the observation group was treated with steaming and douche with traditional Chinese medicine on the basis of control group.The scores of pain and pruritus,the time to fall asleep,and the scores of edema and exudation degree were compared between the two groups before surgery and 3,7 d after surgery.The efficacy of the two groups was compared two months after surgery.Results The scores of pain and pruritus,and the scores of edema and exudation degree of two groups at 3 d after surgery were lower than those before surgery,and the time to fall asleep was shorter than that before surgery,the scores of pain and pruritus,and the scores of edema and exudation degree of two groups at 7 d after surgery were lower than those at 3 d after surgery,and the time to fall asleep was shorter than that at 3 d after surgery,the differences were statistically significant(P<0.05).The scores of pain and pruritus,and the scores of edema and exudation degree of observation group at 7 d after surgery were lower than those of control group at the same time point,and the time to fall asleep was shorter than that of control group at the same time point,the differences were statistically significant(P<0.05).Two months after surgery,the curative effect of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The steaming and douche with traditional Chinese medicine has positive therapeutic effect on anal fistul
