摘  要:目的分析超声引导下颈神经根脉冲射频联合小针刀治疗老年神经根型颈椎病(CSR)的效果。方法抽取2019年2月至2022年1月南阳市第二人民医院收治的老年CSR患者174例,按随机数字表法分为对照组(87例)和观察组(87例)。对照组使用小针刀治疗,观察组使用超声引导下颈神经根脉冲射频联合小针刀治疗。比较两组临床疗效、视觉模拟评分法(VAS)评分、颈椎功能障碍指数(NDI)评分、椎动脉和基底动脉平均流速、颈椎活动度及不良反应发生率。结果观察组总有效率(93.10%,81/87)高于对照组(74.71%,65/87),P<0.05。与对照组比较,治疗后观察组VAS评分、NDI评分较低(P<0.05),椎动脉、基底动脉平均流速较高(P<0.05),颈椎屈伸、旋转活动度较高(P<0.05)。观察组不良反应发生率(5.75%,5/87)与对照组(4.60%,4/87)比较差异未见统计学意义(P>0.05)。结论超声引导下颈神经根脉冲射频联合小针刀治疗老年CSR能提高疗效,缓解患者疼痛症状,改善颈椎功能,还能进一步提升椎动脉、基底动脉平均流速及颈椎活动度,且具有较高安全性。Objective To analyze the effect of ultrasound-guided cervical nerve root pulsed radiofrequency combined with acupotomy therapy on senile cervical spondylotic radiculopathy(CSR).Methods A total of 174 senile CSR patients treated in Nanyang Second People’s Hospital from February 2019 to January 2022 were selected,and they were divided into control group(87 cases)and observation group(87 cases)by random number table method.The control group was treated by acupotomy therapy,and the observation group was treated by ultrasound-guided cervical nerve root pulsed radiofrequency combined with acupotomy therapy.The clinical efficacy,visual analogue scale(VAS)score,neck disability index(NDI)score,average velocity of vertebral artery and basilar artery,range of motion of cervical spine,and incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group(93.10%,81/87)was higher than that of the control group(74.71%,65/87),P<0.05.Compared with the control group,the VAS score and NDI score of the observation group were lower(P<0.05),the average flow velocity of the vertebral artery and basilar artery in the observation group were higher(P<0.05),while the cervical flexion,extension and rotation activities of the observation group were higher after treatment(P<0.05).There was no significant difference in the incidence of adverse reactions between the observation group(5.75%,5/87)and the control group(4.60%,4/87),P>0.05.Conclusions Ultrasound-guided cervical nerve root pulsed radiofrequency combined with acupotomy therapy,in the treatment of senile CSR,can improve clinical efficacy,alleviate the pain symptoms of patients,improve the function of cervical vertebra,and further improve the average flow velocity of vertebral artery and basilar artery and the activity of cervical vertebra,with high safety.
