黄芪桂枝五物汤加味联合硫辛酸治疗糖尿病周围神经病变的疗效及对血脂、神经传导速度的影响

摘　　要：目的:探讨黄芪桂枝五物汤加味联合硫辛酸治疗糖尿病周围神经病变(DPN)患者的疗效及对血脂、神经传导速度的影响。方法:选取2018年1月~2020年1月上海市徐汇区大华医院收治的100例DPN患者,根据乱数表法分为对照组和观察组,各50例。对照组接受硫辛酸治疗,观察组接受黄芪桂枝五物汤加味联合硫辛酸治疗。对比两组患者临床疗效和治疗前后中医症状积分、神经病变严重程度、血糖血脂水平、神经传导速度及不良反应发生情况。结果:观察组临床治疗总有效率明显高于对照组(P<0.05);治疗后观察组中医症状积分、多伦多临床评分系统(TCSS)评分和FBG、2hBG、HbA1c、TC、TG、LDL-C水平明显低于对照组,HDL-C水平、腓总神经和正中神经感觉神经传导速度(SNCV)、运动神经传导速度(MNCV)明显高于对照组(P<0.05)。两组患者在治疗过程中均未出现明显不良反应。结论:黄芪桂枝五物汤加味联合硫辛酸治疗DPN疗效显著,可明显改善血糖血脂水平,加快神经传导速度,且安全性高,值得推广。Objective:To investigate the effect of using Modified Huangqi Guizhi Wuwu Decoction combined with Lipoic Acid in the treatment of patients with diabetic peripheral neuropathy(DPN)and its influence on blood lipids and nerve conduction velocity.Methods:A total of 100 patients with DPN admitted to Dahua Hospital of Xuhui District,Shanghai from January 2,018 to January 2,020 were selected and divided into a controlled group and an observation group,with 50 cases in each group,according to the random number table method.The controlled group received lipoic acid treatment,and the observation group received Huangqi Guizhi Wuwu Decoction combined with Lipoic Acid treatment.The clinical efficacy and TCM symptom scores before and after treatment,the severity of neuropathy,blood glucose and blood lipid levels,nerve conduction velocity,and adverse reactions were compared between the two groups.Results:The total effective rate of clinical treatment in the observation group was significantly higher than that in the controlled group(P<0.05).After treatment,TCM symptom score,Toronto Clinical Scoring System(TCSS)score and FBG,2hBG,HbA1c,TC,TG,and LDL-C levels of the observation group were significantly lower than those of the controlled group,HDL-C levels,nervus peroneus communis and median nerve sensory nerve conduction velocity(SNCV)and motor nerve conduction velocity(MNCV)were significantly higher than those in the controlled group(P<0.05).There were no obvious adverse reactions in the two groups of patients during the treatment.Conclusion:Huangqi Guizhi Wuwu Decoction combined with lipoic acid has a significant effect on the treatment of DPN,which can significantly improve blood sugar and blood lipid levels,accelerate nerve conduction speed,and has high safety.It is worthy of promotion.
