摘　　要：目的了解脑卒中患者健康行为潜在类别及影响因素,为针对不同类别健康行为进行干预提供依据。方法采用方便抽样的方法,于2018年12月—2019年6月,对保定市3所三甲医院381例脑卒中住院患者进行问卷调查。应用潜在类别分析法、多分类Logistic回归模型分析脑卒中患者健康行为潜在类别及影响因素,并探讨健康知识和信念对不同潜在类别健康行为的影响。结果脑卒中患者健康行为分为良好型(19.9%)、低下型(53.3%)和不稳定型(26.8%)3个潜在类别。多分类Logistic回归模型结果显示,农民(OR=10.689)是健康行为不稳定型的危险因素。小学及以下(OR=40.684)、初中(OR=18.105)和高中(OR=19.127)学历及农民(OR=31.886)是健康行为低下型的危险因素。健康知识方面,健康行为良好型得分高于不稳定型和低下型患者(F=28.74,P<0.05)。健康信念方面,健康行为良好型得分高于不稳定型和低下型患者(F=19.79,P<0.05)。结论脑卒中患者健康行为具有明显的分类特征,与健康知识和信念关系密切,提示应对具有不同类别健康行为的脑卒中患者进行针对性干预。Objective To understand the health behavior of stroke patients and the influence factors based on Latent Class Analysis,and develop intervention plan for different class of health behavior.Methods From December 2018 to June 2019,381 stroke patients were selected from three hospitals in Baoding city and investigated by questionnaire.Latent Class Analysis and logistic regression analysis were adopted to analyze the influencing factors of health behavior in different classes stroke patients,and the effects of health knowledge and health belief on different classes of health behavior.Results Stroke patient health behavior could be divided into three classes,including high healthy behavior(19.9%),low health behavior(53.3%)and unstable health behavior(26.8%).Logistic regression analysis showed that the farmer(OR=10.689)was the harmful influencing factor of unstable health beha-vior.Primary school or below(OR=40.684),junior middle school(OR=18.105)and senior high school(OR=19.127),farmer(OR=31.886)were the harmful influencing factors of low health behavior.In terms of the health knowledge,the score of high healthy behavior was higher than that of unstable health behavior and low health beha-vior(F=28.74,P<0.05).In terms of the health belief,the score of patients with high healthy behavior was higher than that of patients with unstable health behavior and low health behavior(F=19.79,P<0.05).Conclusion Stroke patients'health behavior has categorical characteristics,and closely associates with health knowledge and belief,which suggests that interventions should target different types of health behaviors.
