摘 要:目的:系统评价急性缺血性脑卒中病人就医延迟原因,为实施有效的护理干预提供依据。方法:检索PubMed、the Cochrane Library、EMbase、Web of Science、PsycINFO、CINAHL、中国知网、维普数据库、万方数据库、中国生物医学文献数据库等中英文数据库中急性缺血性脑卒中病人就医延迟质性研究的相关文献,检索时限从建库至2022年9月15日。根据澳大利亚JBI循证卫生保健中心质性研究质量评价标准对文献进行质量评价,运用Meta整合方法对研究结果进行整合。结果:共纳入13篇研究,提炼出41个明确的研究结果,归纳整合成知识缺乏与就医方式不当、医疗服务及资源的制约、生活条件的制约、身心感知差异4个结果。结论:就医延迟严重影响急性缺血性脑卒中病人预后及生命安全,医护人员应加强宣教,促进病人及时就医,提高病人的生活质量。Objective:To systematically evaluate the causes of delay in seeking medical treatment in patients with acute ischemic stroke(AIS)and to provide evidence for implementing effective nursing intervention.Methods:Relevant literature of qualitative research on acute ischemic stroke patients′delay in seeking medical treatment was retrieved from PubMed,Cochrane Library,EMbase,Web of Science,PsycINFO,CINAHL,China National Knowledge Network(CNKI),VIP Database,WanFang Database,China Biology Medicine disc(CBMdisc)and other Chinese and English databases,from the establishment of the database to September 15,2022.The quality of the literature was evaluated using the standardized critical appraisal instruments from the Joanna Briggs Institute(JBI),and the meta-integration method was used to integrate the research results.Results:A total of 13 studies were included,and 41 clear research results were extracted.A total of 4 results were summarized and integrated from similar research results:lack of knowledge and inappropriate medical treatment methods,constraints of medical services and resources,constraints of living conditions,and differences in physical and mental perception.Conclusion:Delay in seeking medical treatment seriously affects AIS patients′prognosis and life safety.Medical staff should strengthen publicity and education to promote timely medical treatment and improve patient′s quality of life.
