摘 要:目的:探讨三维CT全脑灌注血容量成像在超急性期脑梗死中的应用效果。方法:针对50例发病时间在6h之内的超急性期脑梗死患者实施常规性的CT血管成像(CTA)和CT平扫检查,对患者CT平扫中相应的表现加以分析,同时应用西门子三维Neuro PBV软件对CTA源图像与CT平扫图像实施相应的减影处理,获取三维彩色PBV图像。针对急性脑缺血灶相应的检出率对CT平扫检查和PBV图像加以比较。所有患者发病以后的2~7d进行CT平扫的复查,针对PBV图像所显现的脑灌注异常区体积和进行CT平扫复查所显现的相应梗死灶体积实施对照分析,同时应用Spearman相关分析方法针对其实施相关性检验。结果:通过实验观察,在全部患者中,第一次进行头颅CT平扫检查,观察到24例患者出现早期脑梗死相关征象,26例患者没有出现异常情况,进行PBV图像的三维重建都观察到与之症状对应的脑灌注异常区域。进行CT平扫的复查,全部患者最终梗死灶体积和PBV图像上相应的灌注异常区域体积之间表现为正相关。结论:通过PBV图像可以以三维的形式对急性脑缺血相关全脑灌注信息加以显现,在早期脑缺血、全方位缺血范围的评估、有效规避病灶漏诊诸多方面都具有十分重要的作用。Objective:To investigate the effect of three-dimensional CT whole brain perfusion volume imaging in hyperacute cerebral infarction.Methods:Routine CT angiography(CTA)and CT plain scan were performed in 50 patients with hyperacute cerebral infarction within 6 hours.The corresponding manifestations in CT plain scan were analyzed.At the same time,Siemens three-dimensional Neuro PBV software was applied.The corresponding subtraction processing was performed for CTA source image and CT plain scan image,and the three-dimensional color PBV image was obtained.The corresponding detection rates of acute cerebral ischemia were compared between CT plain scan and PBV images.All patients were reexamined by CT plain scan 2-7 days after onset.The volume of abnormal cerebral perfusion area shown by PBV image and the volume of corresponding infarct showed by CT plain scan were compared and analyzed.At the same time,Spearman correlation analysis method was used to test the correlation.Results:Through experimental observation,in all patients,the first plain CT scan was performed.24 patients had signs related to early cerebral infarction,and 26 patients had no abnormalities.The abnormal areas of cerebral perfusion corresponding to the symptoms were observed by three-dimensional reconstruction of PBV images.CT scan showed that there was a positive correlation between the final early infarct volume and the corresponding perfusion abnormal area volume on PBV image.Conclusion:PBV images can display the global cerebral perfusion information related to acute cerebral ischemia in a three-dimensional way.It plays a very important role in early cerebral ischemia,the evaluation of all-round ischemic range,and effectively avoiding missed diagnosis of lesions.
