活血解毒饮治疗踝关节骨折切开复位内固定术后患者临床研究

摘　　要：目的:观察活血解毒饮对踝关节骨折切开复位内固定术后患者等速肌力测量指标、骨代谢水平的影响。方法:选取115例踝关节骨折切开复位内固定术患者作为研究对象,采用随机数字表法分为对照组57例和观察组58例。对照组给予西医抗炎、抗感染等常规治疗,观察组在对照组基础上给予活血解毒饮治疗。比较2组骨代谢水平、等速肌力测量指标、炎性因子水平和并发症情况。结果:治疗后,2组血清骨碱性磷酸酶(BALP)、骨钙素(BGP)、Ⅰ型前胶原氨基端延长肽(PⅠNP)水平较治疗前升高(P<0.05),且观察组BALP、BGP、PⅠNP水平高于对照组(P<0.05)。观察组治疗后背伸、趾屈、内翻、外翻的力矩加速能(TAE)、相对峰力矩(PT/BW)、耐力(ER)水平均高于对照组(P<0.05)。治疗后,2组肿瘤坏死因子-α(TNF-α)、C-反应蛋白(CRP)、白细胞介素-1(IL-1)水平较治疗前降低(P<0.05),且观察组TNF-α、CRP、IL-1水平低于对照组(P<0.05)。2组并发症发生率比较,差异无统计学意义(P>0.05)。结论:活血解毒饮治疗踝关节骨折切开复位内固定术后患者效果确切,能够有效改善患者骨代谢水平,提高等速肌力测量指标,降低炎症因子水平。Objective:To observe the effect of Huoxue Jiedu Decoction on indexes of isokinetic muscle strength and levels of bone metabolism of patients with ankle fracture after open reduction and internal fixation.Methods:A total of 115 patients with ankle fracture after open reduction and internal fixation were selected as the study objects and divided into the control group and the observation group,with 57 and 58 cases in each group respectively.The control group was given routine western medicine for antiinflammation and anti-infection,and the observation group was additionally treated with Huoxue Jiedu Decoction based on the treatment of the control group.The levels of bone metabolism,indexes of isokinetic muscle strength,levels of inflammatory factors and incidence of complications were compared between the two groups.Results:After treatment,the levels of serum bone alkaline phosphatase(BALP),osteocalcin(BGP)and typeⅠintact N-terminal propeptide(PⅠNP)in the two groups were increased when compared with those before treatment(P<0.05),and the above three levels in the observation group were higher than those in the control group(P<0.05).After treatment,the torque acceleration energy(TAE),peak torque/body weight(PI/BW)and endurance(ER)level of dorsalis,toe flexion,varus and valgus in the observation group were higher than those in the control group(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),C-reactive protein(CRP)and interleukin-1(IL-1)in both groups were decreased when compared with those before treatment(P<0.05),and the above three levels in the observation group were lower than those in the control group(P<0.05).There was no significant difference being found in the comparison of incidence of complications between the two groups(P>0.05).Conclusion:Huoxue Jiedu Decoction has a definite curative effect on patients with ankle fracture after open reduction and internal fixation,which can effectively improve the levels of bone metabolism of patients,enhance the indexes of isokinetic muscle s
