摘 要:目的探讨非小细胞肺癌患者甲基转移酶DNMT3B基因多态性及其与化疗期间肺部感染易感性的关系。方法选择中国医科大学附属盛京医院2018年10月-2021年10月因非小细胞肺癌住院并接受化疗治疗患者为研究对象,选择在化疗期间出现肺部感染性疾病和未出现肺部感染性疾病患者各60例,分别纳入感染组和非感染组。检测并分析感染组病原菌分布情况,通过电子病历系统收集所有非小细胞肺癌患者性别、年龄、肿瘤分期、肿瘤分化程度和治疗方案情况,参照文献统计感染组患者临床肺炎感染评分(CPIS)、病原检出和感染确诊当天实验室检查指标C-反应蛋白(CRP)、降钙素原(PCT)水平。采用聚合酶链式反应-限制性片段长度多态法检测DNMT3B基因-579位点多态性分布情况。结果哈迪-温伯格检验结果显示,感染组、非感染组DNMT3B基因-579位点均由GG、GT和TT 3种基因型组成,且实际频数和理论频数分布比较差异均无统计学意义,表明所选对象具有较好的群体代表性;感染组非小细胞肺癌患者DNMT3B基因-579位点携带GG基因型和G等位基因频率高于非感染组患者,TT基因型频率低于非感染组患者(P<0.05);感染组携带GG基因型患者CPIS评分、外周血CRP和PCT水平高于携带GT/TT组患者(P<0.05)。结论非小细胞肺癌患者化疗期间肺部感染病原菌主要为革兰阴性菌,DNMT3B基因-579位点携带GG基因型增加非小细胞肺癌患者化疗期间肺部感染易感性,且与严重程度有关,但与感染病原无关。OBJECTIVE To investigate the polymorphism of methyltransferase DNMT3B gene in patients with non-small cell lung cancer and its relationship with susceptibility to lung infection during chemotherapy.METHODS Patients hospitalized for lung cancer and received chemotherapy in Shengjing Hospital affiliated to China Medical University from Oct 2018 to Oct 2021 were enrolled.And 60 patients with pulmonary infectious diseases and 60 patients without pulmonary infectious diseases during chemotherapy were divided into the infection group and non-infected group.The distribution of pathogenic bacteria in the infected group was detected and analyzed.Data including sex,age,tumor stage,tumor differentiation degree and treatment plan in all lung cancer patients were collected by the electronic medical record system,and the clinical pneumonia infection score(CPIS),pathogen detection and laboratory indexes including protein(CRP),procalcitonin(PCT)levels in the patients on the day when they were diagnosed with infection.The polymorphism distribution of DNMT3B gene-579 locus was detected by polymerase chain reaction-restriction fragment length polymorphism method.RESULTS Hardy-weinberg test showed that the-579 locus of DNMT3B gene was composed of GG,GT and TT genotypes in both the infected and non-infected groups,and there was no significant difference between the distribution of the actual frequency and the theoretical frequency,indicating good group representation of the research objects.The frequencies of GG genotype and G allele at the-579 locus of DNMT3B gene in lung cancer patients in the infection group were higher than those in the non-infection group,while the frequency of TT genotype was lower than that in the non-infection group(P<0.05).The CPIS score,peripheral blood CRP and PCT levels in the patients carrying GG genotype in the infection group were significantly higher than those carrying GT/TT group(P<0.05).CONCLUSION The pathogenic bacteria of lung infection in patients with non-small cell lung cancer during chemother
