摘 要:目的探讨中老年患者行主动脉瓣瓣膜置换术选取不同材质瓣膜与术后医院感染的相关性及医院感染造成的经济负担和使用不同瓣膜的经济负担分析。方法选取山东省某三甲教学医院行主动脉瓣瓣膜置换术的中老年患者204例,采用SPSS 23.0统计学软件进行统计学分析分别使用机械瓣膜与生物瓣膜的感染率、感染造成的经济负担及不同瓣膜之间的经济负担。结果使用生物瓣膜的感染率(21.95%)高于使用机械瓣膜的感染率(9.84%),两种瓣膜材质的感染率差异具有统计学意义(P=0.017)。医院感染造成直接经济负担为24000.27元/例。使用生物瓣膜患者住院总费用(165551.51元/例)高于机械瓣膜患者(131179.50元/例)。结论行主动脉瓣膜置换术,中老年患者应优先选择机械瓣膜。OBJECTIVE To investigate the correlation between different material valves for aortic valve replacement and postoperative hospital infection in middle-aged and elderly patients and to analyze the economic burden caused by nosocomial hospital infection by using different valves.METHODS A total of 204 middle-aged and elderly patients who underwent aortic valve replacement in a tertiary care teaching hospital in Shandong Province were recruited.SPSS 23.0 statistical software was used to analyze the infection rates and the economic burden caused by nosocomial infection in patients using different valves.RESULTS The infection rate in patients using biological valve was higher(21.95%)than those using the mechanical valve(9.84%),and the difference in infection rate between the two groups was statistically significant(P=0.017).The direct economic burden of hospital-acquired infections was 24000.27 yuan/case.Total hospital costs in patients with bioprosthetic valves(165551.51 yuan/case)were higher than those in patients with mechanical valves(131179.50 yuan/case).CONCLUSION For aortic valve replacement,mechanical valves should be preferred in middle-aged and elderly patients.
