摘  要:评价口服轮状病毒减毒活疫苗RotaTeq对全球儿童轮状病毒胃肠炎免疫原性,为疫苗的使用提供循证参考。检索2006-2021年公开发表的有关口服轮状病毒减毒活疫苗(Oral Live Attenuated Rotavirus Vaccine,ORV)RotaTeq(RV5)的文献,根据纳入排除标准筛选,共纳入了8 015名研究对象,其中疫苗组纳入研究对象4 062名,对照组纳入研究对象3 953名,使用RevMan5.3软件进行数据分析。共纳入6篇文献,对照组相比疫苗组血清IgA抗体、SNA-G1抗体、SNA-G2抗体、SNA-G3抗体、SNA-G4抗体、SNA-P1A[8]抗体的抗体阳转率分别升高了73%(95%CI:65%~81%)、35%(95%CI:19%~50%)、11%(95%CI:6%~16%)、12%(95%CI:5%~19%)、30%(95%CI:21%~38%)、23%(95%CI:8%~38%);与对照组相比,黄色人种疫苗组血清IgA抗体、SNA-G1抗体、SNA-G2抗体、SNA-G3抗体、SNA-G4抗体、SNA-P1A[8]抗体的抗体阳转率分别升高了75%(95%CI:66%~85%)、41%(95%CI:19%~62%)、14%(95%CI:4%~24%)、19%(95%CI:7%~31%)、28%(95%CI:9%~47%)、34%(95%CI:11%~58%);白色人种中与对照组相比疫苗组血清IgA抗体、SNA-G1抗体、SNA-G2抗体、SNA-G3抗体、SNA-G4抗体、SNA-P1A[8]抗体的抗体阳转率分别升高了82%(95%CI:77%~87%)、54%(95%CI:42%~66%)、15%(95%CI:5%~24%)、9%(95%CI:2%~16%)、38%(95%CI:26%~51%)、22%(95%CI:10%~33%);黑色人种中与对照组相比疫苗组血清IgA抗体、SNA-G1抗体、SNA-G2抗体、SNA-G3抗体、SNA-G4抗体、SNA-P1A[8]抗体的抗体阳转率分别升高了59%(95%CI:52%~67%)、19%(95%CI:14%~24%)、6%(95%CI:2%~10%)、5%(95%CI:1%~9%)、27%(95%CI:19%~36%)、10%(95%CI:5%~16%)。口服轮状病毒减毒活疫苗RotaTeq在不同的人种中免疫原性有一定的差异,且在黄色人种和白色人种中的免疫原性要比黑色人种的免疫原性更好。To evaluate the immunogenicity of oral rotavirus vaccine against rotavirus gastroenteritis in children around the world, and provide an evidence-based reference for the use of vaccines. Literature on oral live attenuated rotavirus vaccine RotaTeq published from 2006 to 2021 were searched, screened, and sorted according to defined exclusion and inclusion criteria. This study included 8,015 subjects;the vaccine group included 4,062 subjects and the control group included 3,953 subjects. Data were analyzed by RevMan 5.3software. Six reports were enrolled. Compared with the control group, the positive conversion rates serum IgA, SNA-G1, SNA-G2, SNA-G3, SNA-G4, and SNA-P1A[8] antibody levels in the vaccine group was increased by 73%(95%CI: 65%-81%), 35%(95% CI: 19%-50%), 11%(95% CI: 6%-16%), 12%(95%CI: 5%-19%), 30%(95% CI: 21%-38%) and 23%(95% CI: 8%-38%);compared with the control group,the positive conversion rates serum IgA, SNA-G1, SNA-G2, SNA-G3, SNA-G4, and SNA-P1A[8] antibody in the yellow race vaccine group was increased by 75%(95% CI: 66%-85%), 41%(95% CI: 19%-62%), 14%(95% CI: 4%-24%), 19%(95% CI: 7%-31%), 28%(95% CI: 9%-47%), and 34%(95%CI: 11%-58%);compared with the control group, the positive conversion rates serum IgA, SNA-G1, SNAG2, SNA-G3, SNA-G4, and SNA-P1A[8] antibody in the white race vaccine group was increased by 82%(95% CI: 77%-87%), 54%(95% CI: 42%-66%), 15%(95% CI: 5%-24%), 9%(95% CI: 2%-16%), 38%(95% CI: 26%-51%), and 22%(95% CI: 10%-33%);compared with the control group,the positive conversion rates serum IgA, SNA-G1, SNA-G2, SNA-G3, SNA-G4, and SNA-P1A [8] antibody in the black race vaccine group was increased by 59%(95% CI: 52%-67%), 19%(95% CI: 14%-24%), 6%(95%CI: 2%-10%), 5%(95% CI: 1%-9%), 27%(95% CI: 19%-36%), and 10%(95% CI: 5%-16%).The immunogenicity of oral rotavirus live attenuated vaccine RotaTeq is different in different races, and the immune effect in yellow and white races is stronger than that in the black race.
