摘　　要：目的 探讨“互联网+”的医院-社区一体化慢性病管理模式对老年高血压患者的护理效果。方法 选择2017年1月—2020年12月河东区向阳楼街社区卫生服务中心诊治的老年高血压患者76例,将患者随机分为对照组和观察组各38例,对照组给予常规护理,观察组应用“互联网+”医院-社区一体化慢性病管理模式管理。比较两组干预前后患者血压水平、高血压知识(使用高血压知识量表评定)及服药依从性评分、生活质量评分及满意度评分。结果 干预后,两组舒张压、收缩压比较均低于干预前,且观察组低于对照组,差异均有统计学意义(P<0.05)。两组患者掌握高血压知识及服药依从性评分均高于干预前,且观察组高于对照组,差异均有统计学意义(P<0.05)。两组生活质量评分高于干预前,且观察组高于对照组,差异均有统计学意义(P<0.05)。观察组护理满意度评分为(96.23±7.89)分,高于对照组的(90.23±8.13)分,差异有统计学意义(t=3.265,P<0.05)。结论“互联网+”医院-社区一体化慢性病管理模式可提高老年高血压患者的护理效果。Objective To investigate the nursing effect of"Internet+"hospital-community integrated chronic disease management mode on elderly patients with hypertension.Methods A total of 76 elderly patients with hypertension who came to Hedong District Xiangyanglou Street Community Health Service Center from January 2017 to December 2020 were selected and randomly divided into control group and observation group with 38 cases in each group.The control group was given routine nursing,and the observation group was managed by"Internet+"hospital-community integrated chronic disease management mode.Blood pressure level,hypertension knowledge(evaluated by hypertension knowledge scale),medication compliance score,quality of life score and satisfaction score were compared between the two groups before and after intervention.Results After the intervention,the diastolic blood pressure and systolic blood pressure in both groups were lower than those before the intervention,and the observation group were lower than the control group,and the differences were statistically significant(P<0.05).The scores of patients'knowledge of hypertension and medication compliance in both groups were higher than those before the intervention,and the observation group were higher than the control group,and the differences were statistically significant(P<0.05).The quality of life scores of both groups were higher than those before the intervention,and the observation group were higher than the control group,and the differences were statistically significant(P<0.05).The nursing satisfaction score of the observation group was(96.23±7.89)points,which was higher than that of the control group with(90.23±8.13)points,and the difference was statistically significant(t=3.265,P<0.05).Conclusion"Internet+"hospital-community integrated chronic disease management mode could improve the nursing effect of elderly patients with hypertension.
