摘  要:目的:建立^(11)C-甲基-N-2β-甲基酯-3β-(4-氟苯基)托烷(CFT)PET图像数据分析的标准空间脑模板及ROIs模板,用于多巴胺转运体(DAT)分布的自动化定量分析。方法:回顾性分析2014年5月至2015年12月在复旦大学附属华山医院检查的16名健康受试者[男3名、女13名,年龄(63.3±6.9)岁]同期采集的脑部^(11)C-CFT PET及MRI T_(1)图像,在MATLAB平台上通过统计参数图(SPM)5进行配准及平滑处理,运用ScAnVp程序建立标准空间脑模板及ROIs模板。继而回顾性分析2014年1月至2019年3月间复旦大学附属华山医院的37名健康受试者[男23名、女14名,年龄(61.7±7.1)岁]及32例帕金森病(PD)患者[男20例、女12例,年龄(61.1±5.4)岁]、10例多系统萎缩帕金森型(MSA-P)患者[男7例、女3例,年龄(60.8±7.1)岁]、10例进行性核上性麻痹(PSP)患者[男5例、女5例,年龄(58.4±6.1)岁]的^(11)C-CFT PET图像,对上述模板进行验证。采用单因素方差分析数据。结果:基于健康受试者11C-CFT PET和MRI T 1图像,建立了用于^(11)C-CFT图像归一化的标准空间脑模板,并通过勾画建立了包含双侧尾状核、前壳核、后壳核(沿长轴方向)及枕叶皮质7个区域的ROIs模板。ROIs模板在各验证组中对位准确。在健康受试者验证组中获得的尾状核、前壳核、后壳核11C-CFT摄取半定量值分别为1.84±0.13、2.18±0.16、1.77±0.11。在PD、MSA-P、PSP各组中^(11)C-CFT摄取半定量值较健康受试者显著减低(F值:49.79~283.83,均P<0.05)。结论:本研究建立的11C-CFT图像分析脑模板具有精准的空间对位,可以为其在临床和科研中的应用提供基础性工具。Objective To establish standard spatial brain template and ROIs template of ^(11)C-methyl-N-2β-carbomethoxy-3β-(4-fluorophenyl)tropane(CFT)PET images for automated quantitative analysis of dopamine transporter(DAT)distribution.Methods From May 2014 to December 2015,11C-CFT PET and MRI T_(1) brain images of 16 healthy volunteers(3 males,13 females;age(63.3±6.9)years)from Huashan Hospital,Fudan University were co-registered and smoothed using statistical parametric mapping(SPM)5 software based on MATLAB to create a standard spatial brain template.The ROIs template was established by ScAnVp procedures.These templates were clinically verified by using ^(11)C-CFT PET images of 37 healthy volunteers(23 males,14 females;age(61.7±7.1)years),32 Parkinson′s disease(PD)patients(20 males,12 females;age(61.1±5.4)years),10 multiple system atrophy with predominant parkinsonism(MSA-P)patients(7 males,3 females;age(60.8±7.1)years)and 10 progressive supranuclear palsy(PSP)patients(5 males,5 females;age(58.4±6.1)years)from Huashan Hospital,Fudan University between January 2014 and March 2019.One-way analysis of variance was used to analyze data.Results Based on the ^(11)C-CFT PET images and MRI T1 images of healthy volunteers,a standard spatial brain template for normalization of ^(11)C-CFT PET images was created.The ROIs template was established including seven regions:bilateral caudate,anterior putamen,posterior putamen(along the long axis)and the occipital cortex.The ROIs template was accurately aligned in each verification group.The normal reference values of semi-quantitative DAT distribution in caudate,anterior putamen and posterior putamen were obtained(1.84±0.13,2.18±0.16,1.77±0.11).The semi-quantitative values of ^(11)C-CFT uptake in each ROI in patients were significantly lower than those in healthy volunteers(F values:49.79-283.83,all P<0.05).Conclusion The established brain templates with accurate spatial alignment for ^(11)C-CFT image analysis can provide foundational tools for the application of ^(11)C-CFT
