犬特发性血小板减少性紫癜的中西医结合治疗

摘　　要：犬特发性血小板减少性紫癜(ITP)是犬严重血小板减少症中常见的类型。采用西兽药治疗后,短期内患犬存活率能达到80%,但药物减量后有一定的复发率。接诊1例犬特发性血小板减少性紫癜的病例,同时有其他病史以及长期用药史,并在服用泼尼松龙后出现高血糖。患犬精神沉郁,喜阴凉地,四肢偏热,皮肤有出血点、出血斑;口腔内也有鲜红色出血点,舌质红绛,上有裂纹,脉细数;粪黑,尿色赤。因多病久治,五脏受损,气阴不足,属气虚血热型血证。以凉血止血、清热生津、滋阴补气为治法组方。方中重用生石膏清热生津,人参、生黄芪、生白术补气,侧柏叶、炒荷叶、藕节凉血止血,麦冬、玄参配伍以养阴生津,丹参、远志、炙甘草补心,并加入温性的补血药当归、熟地,以防清热过度使机体生寒,而后根据具体情况加减用药调方3次。经中西医结合治疗后,患犬明显好转,且长期脱毛症状也明显改善,至回访日未见复发。中西医结合有助于高效治疗犬特发性血小板减少性紫癜,可为临床诊疗提供参考。Canine idiopathic thrombocytopenic purpura is a common type of severe thrombocytopenia in dogs.After western medical treatment,the survival rate of the dogs can reach 80%in the short term,but there is a certain recurrence rate after the medication reduction.A case of canine idiopathic thrombocytopenic purpura in a dog was admitted to our hospital.The dog had a history of other disease and a long-term medication history.The dog also had hyperglycemia after taking prednisolone.At the time of the visit to the Chinese veterinary clinic,the dog was depressed,preferring shady places,and had hot limbs.The skin bleeding spots were red in color,and there were also bright red bleeding spots in the mouth.The tongue was red and vivid,with cracks on it.And the pulse was thready and rapid.After prolonged treatment of multiple diseases,the five Zang were damaged and there was a deficiency of Qi and Yin,which was a pattern of Qi deficiency and blood heat.The formula was based on cooling the blood to stop bleeding,clearing heat to produce fluid,nourishing Yin and Qi.The formula was used to clear heat and generate fluid,replenish Qi,cool the blood and stop bleeding,nourish Yin and generate fluid,nourish the heart.And then the formula was adjusted three times by adding and subtracting herbs according to the specific situation in the treatment.After the treatment of integrated traditional Chinese veterinary medicine(TCVM)and western medicine in our hospital,the acute clinical symptoms such as bleeding spots and melena on the skin were significantly improved,as well as the chronic symptoms typical of long-term alopecia.And no recurrence was observed until the return visit date.This case suggests that the integration of TCVM and western medicine can help to treat canine idiopathic thrombocytopenic purpura effectively,which provides a reference for clinical veterinarians in the diagnosis and treatment.
