摘 要:目的评估VITEK-MS质谱仪联合血清分离胶促凝管快速直接鉴定阳性血培养标本中微生物的可行性与临床价值。方法收集2021年9月—2022年6月血培养报阳标本333份,使用血清分离胶管分离、洗涤、纯化待测菌,采用VITEK-MS对待测标本进行菌种鉴定,与转种纯培养菌落VITEK-MS鉴定结果比较,评价该方法的鉴定符合率。结果333份阳性血培养瓶有303例真阳性瓶和30例假阳性瓶。303份培养阳性的标本,其正确鉴定率为77.23%(234/303),错误鉴定率为3.96%(12/303),鉴定无结果为18.81%(57/303),种鉴定符合率为77.23%(234/303),属鉴定符合率为80.19%(243/303),30份培养阴性标本正确鉴定率为100.00%(30/30),全部标本正确鉴定率为79.28%(264/333)。结论VITEK-MS质谱仪联合血清分离胶促凝管能准确和快速地鉴定血流感染常见菌,从而缩短血流感染诊断时间,为临床上快速、合理选用抗生素提供依据,且操作简便,成本低廉,具有临床应用价值。Objective This paper aims to evaluate the feasibility and clinical value of VITEK-MS mass spectrometer combined with serum separation gel-promoting tube for rapid and direct identification of microorganisms in positive blood culture specimens.Methods A total of 333 positive specimens in blood culture were collected from September 2021 to June 2022.The bacteria to be tested were separated,washed and purified by serum separation hose,and then identified by VITEK-MS.The identification coincidence rate of this method was evaluated by comparing it with the results of VITEK-MS.Results There were 303 true positive specimens and 30 false positive specimens in the 333 positive specimens in blood culture.Among the 303 positive culture specimens,the correct identification rate was 77.23%(234/303),the false identification rate was 3.96%(12/303),the invalid identification result was 18.81%(57/303),and the consistency rate of species identification was 77.23%(234/303).The consistency rate of genus identification was 80.19%(243/303),the correct identification rate of 30 culture negative specimens was 100.00%(30/30),and the correct identification rate of all specimens was 79.28%(264/333).Conclusion The VITEK-MS mass spectrometer combined with serum separation gel coagulation tube can accurately and quickly identify common bacteria in bloodstream infections,thereby shortening the diagnosis time of bloodstream infections,providing a basis for rapid and reasonable selection of antibiotics in clinical practice.This method is easy to operate,low in cost,and has clinical values.
