预后营养指数对初治涂阳肺结核患者疗效的预测价值

摘　　要：目的研究初治涂阳肺结核患者不同疗效下的预后营养指数(PNI)结果,探讨PNI对涂阳肺结核疗效预测价值。方法回顾2020年1月—2022年6月湖州市中心医院诊治的438例活动性肺结核患者病历资料,根据痰涂结果分为涂阳组246例、涂阴组192例;涂阳病例根据疗效分为治愈组168例、非治愈组59例。收集所有患者的临床资料及血液检测指标包括血清白蛋白、淋巴细胞计数、CD4^(+)T淋巴细胞比例、PNI,进行组间比较并绘制受试者工作曲线。结果对照组、涂阴组和涂阳组之间的血清白蛋白、淋巴细胞计数、CD4^(+)T淋巴细胞比例、PNI各组间进行比较,差异均有统计学意义(P<0.05);治愈组血清白蛋白、淋巴细胞计数、CD4^(+)T淋巴细胞比例、PNI与非治愈组之间进行比较,差异有统计学意义(P<0.05);受试者工作曲线结果显示,PNI相对于血清白蛋白、淋巴细胞计数、CD4^(+)T淋巴细胞比例对涂阳患者具有更高的疗效预测价值。结论涂阳肺结核患者的营养指标、免疫指标、PNI均有所下降,PNI对涂阳肺结核患者疗效具有良好的诊断预测价值。Objective This paper aims to study the results of the prognostic nutrition index(PNI)under different efficacy of patients with newly diagnosed smear positive tuberculosis and explore the predictive value of PNI on the treatment of smear positive tuberculosis.Methods The medical records of 438 patients with active pulmonary tuberculosis diagnosed and treated in Huzhou Central Hospital from January 2020 to June 2022 were reviewed.According to the smear results of sputum,they were divided into the smear positive group(n=246)and the smear negative group(n=192).According to curative effect,smear positive patients were divided into cured group(n=168)and non-cured group(n=59).Clinical data and blood test indicators,including serum albumin,lymphocyte count,CD4^(+)T lymphocyte ratio,and PNI,were collected from all patients for inter-group comparison and receiver working curve was drawn.Results There was statistical significance on the differences in serum albumin,lymphocyte count,CD4^(+)T lymphocyte ratio and PNI among control group,smear negative group and smear positive group(P<0.05).Serum albumin,lymphocyte count,CD4^(+)T lymphocyte ratio and PNI were compared between cured group and non-cured group,and the differences were statistically significant(P<0.05).The results of receiver working curve showed that PNI had higher predictive value than serum albumin,lymphocyte count and CD4^(+)T lymphocyte ratio in smear positive patients.Conclusion The nutrition indicators and immune indicators and PNI for patients with smear positive tuberculosis have declined.PNI has good diagnostic predictive value for patients with smear positive tuberculosis.
