经尿道输尿管镜钬激光碎石术治疗儿童输尿管下段结石及膀胱结石的临床效果

摘　　要：目的:探讨经尿道输尿管镜钬激光碎石术治疗儿童输尿管下段结石及膀胱结石的临床效果。方法:选取2016年1月—2022年10月广西医科大学第八附属医院收治的60例输尿管下段结石及膀胱结石患儿。根据随机数表法将其分为研究组和对照组,各30例。对照组给予传统开放手术治疗,研究组给予经尿道输尿管镜钬激光碎石术治疗。比较两组结石清除率,围手术期指标,术前及术后1个月肾功能指标及术后并发症。结果:研究组结石清除率(96.67%)与对照组(93.33%)比较差异无统计学意义(P>0.05)。研究组术中出血量少于对照组,手术时间、住院时间均短于对照组,术后进食时间早于对照组,差异有统计学意义(P<0.05)。术后1个月,两组胱抑素C(Cys C)、血尿素氮(BUN)、中性粒细胞明胶酶相关脂质运载蛋白(NGAL)水平均降低,研究组Cys C、BUN、NGAL水平均低于对照组,差异有统计学意义(P<0.05)。研究组术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:儿童输尿管下段结石及膀胱结石采用经尿道输尿管镜钬激光碎石术治疗具有较高的结石清除率,能够有效改善围手术期指标及肾功能,减少术后并发症。Objective:To explore the clinical effect of transurethral ureteroscopic holmium laser lithotripsy in the treatment of lower ureteral calculi and bladder calculi in children.Method:A total of 60 children with lower ureteral calculi and bladder calculi admitted to the Eighth Subsidiary Hospital of Guangxi Medical University from January 2016 to October 2022 were selected.According to the random number table method,they were divided into study group and control group,with 30 cases in each group.The control group was treated with traditional open surgery,and the study group was treated with transurethral ureteroscopic holmium laser lithotripsy.The stone clearance rate,perioperative indicators,renal function indicators before and 1 month after surgery and postoperative complications were compared between the two groups.Result:There was no significant difference in stone clearance rate between the study group(96.67%)and the control group(93.33%)(P>0.05).The amount of intraoperative blood loss in the study group was less than that in the control group,the operation time and hospital stay were shorter than those in the control group,and the time of postoperative feeding was earlier than that in the control group,the differences were statistically significant(P<0.05).One month after surgery,the levels of cystatin C(Cys C),blood urea nitrogen(BUN)and neutrophil gelatinase-associated lipocalin(NGAL)in the two groups decreased,and the levels of Cys C,BUN and NGAL in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).The incidence of postoperative complications in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Transurethral ureteroscopic holmium laser lithotripsy treatment for lower ureteral calculi and bladder calculi in children has a high stone clearance rate,which can effectively improve perioperative indicators and renal function,and reduce postoperative complications.
