口腔专科类重点病种临床质量与绩效管理应用探索

摘　　要：目的通过构建以“重点病种”为抓手、以“绩效激励”为动力点的口腔专科重点病种绩效管理模式,强化口腔专科医院诊治口腔疑难病例的功能定位,提升医院综合竞争力。方法采用大数据分析、文献回顾以及专家咨询等方法确定重点病种目录;基于医疗费用、诊疗时间、资源消耗等相关因素测定各重点病种权重RW值,在临床应用管理中不断完善。结果通过病种-病例-绩效管理实践,2018—2021年全院临床学科重点病种完成总数量853例,累计RW总值5276.51,覆盖医院7个主要科室和19个重点病种,病种难度和质量逐年提升。结论基于重点病种的绩效管理,有利于促进口腔专科医院临床业务结构优化,助力医院内涵质量建设,提升医院管理水平。Objective To establish a performance management model with“key diseases”as the starting point,“excellent cases”as the basis,and“performance incentives”as the driving force to strengthen the functional positioning of the tertiary specialty hospital for the diagnosis and treatment of difficult oral cases,and to enhance the overall competitiveness of the hospital.Methods Big data analysis,literature review and expert consultation were used to determine the catalogue of key diseases;the relative weight(RW)value of each key disease was determined based on related factors such as medical expenses,time and resource consumption,and it was continuously improved in clinical application management.Results Through the practice of disease-case-performance management,from 2018 to 2021,the total number of key diseases in clinical disciplines in the hospital was 853,with a cumulative RW value of 5276.51,covering 7 major departments and 19 key diseases in the hospital,and the difficulty and quality of diseases have been improved year by year.Conclusion Performance management based on key diseases is conducive to promoting the optimization of the clinical business structure of stomatology specialist hospitals,assisting the construction of hospital connotation and quality,and improving hospital management level.
