达格列净治疗糖尿病合并高尿酸血症老年患者的效果及对血糖、尿酸的影响

摘　　要：目的:探讨达格列净治疗糖尿病合并高尿酸血症(HUA)老年患者的效果及对血糖、尿酸的影响。方法:选取2021年1—9月重庆市长寿区人民医院收治的80例2型糖尿病(T2DM)合并HUA老年患者。根据随机数表法将其分为对照组及试验组,各40例。对照组于入院后给予常规治疗,试验组在此基础上给予达格列净片。比较两组治疗前及治疗4周、8周及12周血糖指标、尿酸、血脂指标、炎症因子。结果:治疗8周及12周,试验组空腹血糖(FPG)、餐后2 h血糖(2 h PG)及糖化血红蛋白(HbA1c)水平均低于对照组,差异有统计学意义(P<0.05)。治疗8周及12周,试验组血尿酸(SUA)水平均低于对照组,24 h尿尿酸(24 h UUA)水平均高于对照组,差异有统计学意义(P<0.05)。治疗8周及12周,试验组总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)水平均低于对照组,高密度脂蛋白胆固醇(HDL-C)水平高于对照组,差异有统计学意义(P<0.05)。治疗8周及12周,试验组白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平均低于对照组,差异有统计学意义(P<0.05)。结论:达格列净用于T2DM合并HUA老年患者治疗中,可有效控制血糖,促进UA排泄,并改善患者血脂指标,有利于减轻机体炎症反应。Objective:To investigate the effect of Dapagliflozin in the treatment of elderly patients with diabetes mellitus complicated with hyperuricemia(HUA)and its influence on blood glucose and uric acid.Method:From January to September 2021,80 elderly patients with type 2 diabetes mellitus(T2DM)complicated with HUA who admitted to People's Hospital of Changshou District of Chongqing were selected.According to the random number table method,they were divided into control group and experimental group,40 cases in each group.The control group was given routine treatment after admission,and the experimental group was given Dapagliflozin Tablets on this basis.Blood glucose index,uric acid index,blood lipid index and inflammatory factors were compared between the two groups before treatment,4 weeks,8 weeks and 12 weeks of treatment.Result:At 8 weeks and 12 weeks of treatment,the levels of fasting plasma glucose(FPG),2 h postprandial plasma glucose(2 h PG)and glycosylated hemoglobin(HbA1c)in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).At 8 weeks and 12 weeks of treatment,the blood uric acid(SUA)level of the experimental group was lower than that of the control group,and the 24 h urinary uric acid(24 h UUA)level was higher than that of the control group,and the differences were statistically significant(P<0.05).At 8 weeks and 12 weeks of treatment,the levels of total cholesterol(TC),triglyceride(TG)and low density lipoprotein cholesterol(LDL-C)in the experimental group were lower than those in the control group,and the level of high density lipoprotein cholesterol(HDL-C)in the experimental group was higher than that in the control group,and the differences were statistically significant(P<0.05).At 8 weeks and 12 weeks of treatment,the levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Dapagliflozin
