前臂旋前、旋后桡骨头挤压屈肘复位法治疗儿童桡骨头半脱位的效果研究

摘　　要：目的:探讨前臂旋前、旋后桡骨头挤压屈肘复位法治疗儿童桡骨头半脱位的效果。方法:纳入2019年5月—2022年5月汕头市澄海区人民医院收治的80例儿童桡骨头半脱位患儿,随机分为对照组和研究组,各40例。对照组使用前臂旋后桡骨头挤压屈肘复位法治疗,研究组使用前臂旋前桡骨头挤压屈肘复位法治疗。比较两组复位、再脱位情况,比较两组疼痛程度、肘关节功能恢复情况及患儿家属满意度。结果:两组患儿全部复位成功,研究组一次复位成功率高于对照组,再脱位率低于对照组,差异均有统计学意义(P<0.05)。研究组中度、重度疼痛患者均少于对照组,轻度疼痛患者多于对照组,差异有统计学意义(P<0.05)。研究组肘关节功能优良率、患儿家属总满意度高于对照组,差异有统计学意义(P<0.05)。结论:前臂旋前桡骨头挤压屈肘复位法治疗儿童桡骨头半脱位,能有效提高一次复位成功率,减少再脱位,减轻疼痛程度,肘关节功能恢复更好,患儿家属满意度更高。Objective:To investigate the effect of forearm pronation and supination radial head extrusion elbow flexion reduction method in the treatment of children with radial head suxation.Method:Eighty children with radial head subluxation who treated in Shantou City Chenghai District People's Hospital from May 2019 to May 2022 were enrolled,they were randomly divided into the control group and the study group,40 cases in each group.The control group was treated with forearm supination radial head extrusion elbow flexion reduction method,and the study group was treated with forearm pronation radial head extrusion elbow flexion reduction method.The reduction and redislocation of two groups were compared,and the degree of pain,recovery of elbow joint function and satisfaction of child's family members between two groups were compared.Result:All children in two groups were successfully repositioned,the success rate of one-time reduction in the study group was higher than that in the control group,and the rate of re-dislocation was lower than that in the control group,the differences were statistically significant(P<0.05).The patients with moderate and severe pain in the study group were less than those in the control group,and patients with mild pain were more than those in the control group,the differences were statistically significant(P<0.05).The excellent and good rate of elbow joint function and total satisfaction of family members in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Forearm pronation radial head extrusion elbow flexion reduction method in the treatment of children with radial head suxation can effectively improve the success rate of of one-time reduction,reduce re-dislocation,reduce the degree of pain,elbow joint function recovery is better,the children's family satisfaction is higher.
