摘　　要：目的比较大黄不同用法联合芒硝外敷对重症急性胰腺炎患者肠道功能和炎性因子的影响。方法选取2020年10月-2022年1月上饶市广信区人民医院收治的90例SAP患者,随机分为对照组、口服组、灌肠组各30例。比较三组患者的肠道功能、血清C反应蛋白、血清淀粉酶以及不良反应。结果灌肠组的腹痛缓解时间、腹胀缓解时间、首次排气时间、肠鸣音恢复时间均短于对照组与口服组(P<0.05);口服组的腹胀缓解时间、肠鸣音恢复时间短于对照组(P<0.05)。灌肠组治疗后的血清C反应蛋白、血清淀粉酶水平均低于对照组与口服组(P<0.05);口服组治疗后的血清C反应蛋白水平低于对照组(P<0.05)。三组患者食欲不振、恶心、头晕的发生率比较,差异无统计学意义(P>0.05)。结论相比大黄口服治疗,大黄灌肠联合芒硝外敷治疗重症急性胰腺炎更能改善肠道功能、减轻机体炎症反应。Objective To compare the effects of different usages of rhubarb combined with mirabilite external application on intestinal function and inflammatory factors in patients with severe acute pancreatitis.Methods 90 SAP patients admitted to our hospital from October 2020 to January 2022 were randomly divided into control group,oral group and enema group with 30 cases each.The intestinal function,serum C-reactive protein,serum amylase and adverse reactions of the three groups were compared.Results The abdominal pain relief time,abdominal distension relief time,first exhaust time and intestinal sound recovery time in enema group were shorter than those in control group and oral group(P<0.05).The relief time of abdominal distension and recovery time of intestinal sound in oral group were shorter than those in control group(P<0.05).The levels of serum C-reactive protein and serum amylase in enema group were lower than those in control group and oral group(P<0.05).The serum C-reactive protein level in the oral group was lower than that in the control group(P<0.05).There was no significant difference in the incidence of loss of appetite,nausea and dizziness among the three groups(P>0.05).Conclusion Compared with rhubarb oral therapy,rhubarb enema combined with mirabilite external application in the treatment of severe acute pancreatitis can improve intestinal function and reduce inflammation.
