慢性下腰痛伴下肢神经根病变患者的肌肉激活时间和平衡反应能力分析

摘　　要：目的:探讨慢性下腰痛(CLBP)伴下肢神经根病变患者的肌肉激活时间和平衡反应能力。方法:随机纳入人口学资料相匹配的两组人群:CLBP伴下肢神经根病变患者30例,作为CLBP-R组;健康人群30例,作为对照组。两组人群均接受一系列的平衡干扰测试(包括双臂快速抬高、突发性加载负荷、脚趾站立试验),观察其肌肉激活的时间和序列,以及相应的动力学反应情况。结果:与对照组相比,CLBP-R组患者在双臂快速抬高试验中的下肢肌肉激活明显延迟(P<0.05);在突发性负荷试验中,CLBP-R组患者的肌肉激活时间没有延迟,但在后链肌肉激活序列上存在明显差异(P<0.05)。在突发性负荷试验中,CLBP-R组患者表现出较少的前后方运动,但在脚趾站立试验中,患者表现出较大的内侧向移动量。结论:CLBP伴下肢神经根病变患者在肌肉激活时间、序列和总体平衡控制方面与健康人群有显著差异,尤其是下肢的差异较显著,表明神经根病变可能在改变此类患者的平衡控制方面发挥作用。Objective:To investigate the muscle activation time and balance response ability of CLBP patients with lower extremity radiculopathy.Methods:30 CLBP patients with lower extremity radiculopathy were randomly selected as CLBP-R group and 30 healthy people as control group.Both groups received a series of balance interference tests(including rapid raising of arms,sudden loading,toe standing test),to observe the time and sequence of muscle activation and the corresponding dynamic response.Results:Compared with the control group,the lower limb muscle activation of CLBP-R group was significantly delayed(P<0.05);in the sudden load test,the muscle activation time of CLBP-R group was not delayed,but there was significant difference in the activation sequence of posterior chain muscle(P<0.05).In the sudden load test,the CLBP-R group showed less anteroposterior movement,but in the toe standing test,the patients showed greater medial movement.Conclusion:There are significant differences in muscle activation time,sequence and overall balance control between CLBP patients with lower extremity radiculopathy and healthy people,especially in the lower extremities,indicating that radiculopathy may play a role in changing the balance control of these patients.
