摘  要:目的探讨血清中三磷腺苷合酶抑制因子1(ATPIF1)对2型糖尿病(T2DM)的诊断效能。方法选择2021年5月至2021年9月新乡医学院第三附属医院内分泌科收治的80例T2DM患者为研究对象(T2DM组);另选择同期80例体检健康者为健康对照组。采集2组患者空腹外周静脉血,应用己糖激酶法检测血清血糖(GLU)水平,氧化酶法检测血清总胆固醇(TC)、三酰甘油(TG)水平,直接法检测血清高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)水平,酶联免疫吸附法检测血清中胰岛素(INS)、C-肽和ATPIF1水平,离子交换高效液相色谱法检测糖化血红蛋白(HbA1c)水平,实时荧光定量聚合酶链式反应法检测外周血单核细胞中ATPIF1 mRNA相对表达量。采用受试者操作特征(ROC)曲线下面积(AUC)评估血清ATPIF1水平、外周血单核细胞ATPIF1 mRNA相对表达量诊断T2DM的效能。结果T2DM组患者的GLU、HbA1c、INS和C-肽水平显著高于健康对照组,血清ATPIF1水平和外周血单核细胞ATPIF1 mRNA相对表达量显著低于健康对照组(P<0.05);2组受试者的TC、TG、HDL-C、LDL-C水平比较差异无统计学意义(P>0.05)。血清ATPIF1预测T2DM的AUC为0.916(95%置信区间:0.873~0.960,P<0.05);当ATPIF1以1.37μg·L^(-1)为截断点时,Youden指数为0.738,敏感度为93.8%,特异度为80.0%。外周血单核细胞中ATPIF1 mRNA相对表达量预测T2DM的AUC为0.965(95%置信区间:0.943~0.988,P<0.05);当ATPIF1 mRNA相对表达量以0.95为截断点时,Youden指数为0.838,敏感度为83.8%,特异度为100.0%。血清ATPIF1水平和外周血单核细胞ATPIF1 mRNA相对表达量预测T2DM的Youden指数显著低于GLU和HbA1c,显著高于INS和C-肽(P<0.001)。结论血清ATPIF1水平和外周血单核细胞中ATPIF1 mRNA表达水平降低会增加T2DM的患病风险,可作为诊断T2DM的生物标志物。Objective To investigate the diagnostic efficacy of adenosine triphosphate synthase inhibitory factor 1(ATPIF1)in type 2 diabetes mellitus(T2DM).Methods A total of 80 patients with T2DM admitted to the Department of Endocrinology of the Third Affiliated Hospital of Xinxiang Medical University from May 2021 to September 2021 were selected as the research subjects(T2DM group).In addition,80 cases of healthy people with physical examination during the same period were selected as the healthy control group.Fasting peripheral venous blood were collected from subjects in the two groups.The serum glucose(GLU)was detected by hexokinase method,the total cholesterol(TC)and triacylglycerol(TG)levels in serum were detected by oxidase method;the high-density lipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDL-C)levels in serum were detected by direct method;the insulin(INS),C-peptide and ATPIF1 levels in serum were detected by enzyme-linked immunosorbent;the glycosylated hemoglobin(HbA1c)level was detected by ion exchange high performance liquid chromatography;the expression of ATPIF1 mRNA in peripheral blood mononuclear cells was detected by real-time polymerase chain reaction.The efficacy of serum ATPIF1 level and the relative expression of ATPIF1 mRNA in peripheral blood mononuclear cells in the diagnosis of T2DM was evaluated by area under curve(AUC)of receiver operating characteristic(ROC)curve.Results The levels of GLU,HbA1c,INS and C-peptide of patients in the T2DM group were significantly higher than those in the healthy control group,and the serum ATPIF1 level and the relative expression of ATPIF1 mRNA in peripheral blood mononuclear cells were significantly lower than those in the healthy control group(P<0.05);there was no significant difference in TC,TG,HDL-C and LDL-C levels of subjects between the two groups(P>0.05).The AUC of serum ATPIF1 predicting T2DM was 0.916,when ATPIF1 was at 1.37μg·L^(-1)as the cut-off point,the Youden index was 0.738,the sensitivity was 93.8%,and the specificity
