晕针起效引发的对针刺疗法的思考

摘　　要：[目的]探究晕针起效的要素和临床特征,为针灸临床提高针刺疗效,实施安全针刺提供一定思路。[方法]收集晕针的古今文献,对针刺晕针起效进行梳理,分析古今文献中晕针起效的病例、针刺部位以及针刺操作特点,总结机体因素在针刺疗效中的作用以及自主神经在针刺效应产生中的重要性,从针刺有效刺激量、针刺治病“因人制宜”原则、安全针刺等方面展开讨论。[结果]晕针起效好发的群体是女性患者,以再次针灸者为多,病种以慢性疼痛性疾病、自主神经系统功能失调的疾病为主,涉及的腧穴多位于神经血管丰富的部位。针刺手法是针灸治疗的核心环节,刺激量是针刺获效的关键,针刺晕针起效现象与患者个体形质、心理、生理特征、病理情况存在密切联系。[结论]针灸医师不能盲从古今文献报道的晕针起效的案例,在临床上为了追求疗效,而故意促发晕针,而需因人、因病、因部位制宜,实施合适的针刺刺激,实现安全而有效的针刺治疗。[Objective]To investigate the elements and clinical characteristics of acupuncture syncope to provide some ideas to improve the efficacy of acupuncture and implement safe acupuncture in clinics.[Methods]Review the ancient and modern literatures on the efficacy accompanied acupuncture syncope,analyze the characteristics of cases of fainting,acupuncture sites and acupuncture operations,summarize the role of organic factors in the efficacy of acupuncture and the importance of the autonomic nerve in the production of acupuncture effects,and then discuss the effective amount of acupuncture stimulation,the principle of“tailoring to the individual”acupuncture treatment and safe acupuncture.[Results]The most common group of patients who experience the efficacy accompanied with acupuncture syncope are females who are more likely to return to acupuncture,and who suffer from chronic pain and dysfunction of the autonomic nerve system,with the acupoints involved being mainly neurovascular.The acupuncture technique is the core of acupuncture treatment,and the amount of stimulation is the key to the effectiveness of acupuncture.The phenomenon of acupuncture syncope is closely related to the individual patient's physical,psychological and physiological characteristics and pathology.[Conclusion]Acupuncturists should not blindly follow the cases reported in ancient and modern literatures on acupuncture syncope taking effect,and deliberately promote acupuncture syncope in clinical practice in pursuit of efficacy,but need to implement appropriate acupuncture stimulation to achieve safe and effective acupuncture treatment,depending on the person and the site of the disease.
