摘 要:目的探讨与分析肾移植术后肺部感染患者淋巴细胞亚群检测的临床意义。方法选择2020年1月—2022年5月在广西医科大学第二附属医院移植医学中心完成肾移植的50例术后患者作为研究对象,检测淋巴细胞亚群相对含量及绝对含量。所有患者术后给予免疫抑制剂治疗,随访肺部感染发生情况并进行相关性分析。结果在50例患者中,术后6个月发生感染12例(感染组),占比24.00%;在感染的病原体中,肺炎克雷伯菌4例,铜绿假单胞菌4例、鲍曼不动杆菌2例、大肠埃希菌1例,白假丝酵母菌1例。感染组患者的CD3^(+)、CD4^(+)淋巴细胞亚群相对含量、绝对含量明显低于非感染组(P<0.05),CD8^(+)淋巴细胞亚群相对含量、绝对含量明显高于非感染组(P<0.05)。Pearson分析显示CD3^(+)、CD4^(+)、CD8^(+)淋巴细胞亚群相对含量、绝对含量与肺部感染的发生存在相关性(P<0.05)。二分类logistic回归分析显示CD3^(+)、CD4^(+)、CD8^(+)淋巴细胞亚群相对含量、绝对含量为影响肺部感染发生的重要因素(P<0.05)。结论肾移植术后肺部感染比较常见,CD3^(+)、CD4^(+)、CD8^(+)淋巴细胞亚群相对含量、绝对含量与肺部感染的发生存在相关性,淋巴细胞亚群检测能有效预测患者肺部感染的发生。Objective To explore and analysis the clinical significance of detection of lymphocyte subsets in patients with pulmonary infection after kidney transplantation.Methods From January 2020 to May 2022,50 cases of postoperative patients who completed kidney transplantation in the Transplantation Center of the Second Affiliated Hospital of Guangxi Medical University were selected as the research subjects,and the content of lymphocyte subsets was detected.All patients were given immunosuppressive therapy after operation,and the incidence of pulmonary infection was followed up and correlation analysis was performed.Results Among the 50 patients,12 cases(infection group)developed infection 6 months after surgery,accounting for 24.0%;Four pathogens infected were found(Klebsiella pneumoniae in 4 cases,Pseudomonas aeruginosa in 4 cases,Acinetobacter baumannii in 2 cases,Escherichia coli in 1 case and Candida albus in 1 case).The relative contents and absolute content of CD3^(+)and CD4^(+)lymphocyte subsets in the infection group were significantly lower than those in the non-infection group(P<0.05),and the relative contents and absolute content of CD8^(+)lymphocyte subsets were significantly higher than those in the non-infection group(P<0.05).Pearson analysis showed that the relative contents of CD3^(+),CD4^(+),CD8^(+)lymphocyte subsets and absolute content were correlated with the occurrence of pulmonary infection(P<0.05).Binary logistic regression analysis showed that the relative content and absolute content of CD3^(+),CD4^(+),CD8^(+)lymphocyte subsets were the important factors that affected the occurrence of pulmonary infection(P<0.05).Conclusion Pulmonary infection is common after kidney transplantation.The relative content of CD3^(+),CD4^(+),and CD8^(+)lymphocyte subsets are correlated with the occurrence of pulmonary infection.The detection of lymphocyte subsets can effectively predict the occurrence of pulmonary infection in patients.
