摘　　要：目的:观察密集型银质针导热疗法对肩胛部慢性软组织损伤的临床疗效。方法:将78例肩胛部慢性软组织损伤患者随机分为治疗组和对照组,每组各39例,治疗组予密集型银质针导热疗法治疗患者肩胛区压痛点(脊柱旁开3寸向外侧布针4行3列,针距约1.5 cm,每一行有3根银质针,留针20 min,1次/周)。对照组局部外贴氟比洛芬凝胶贴膏治疗(8 h/次,1次/d,连续1周)。两组均治疗1周后休息1周,连续治疗2周为1个疗程,连续治疗2个疗程。观察两组患者治疗前后及治疗后3个月随访时疼痛视觉模拟评分(VAS)及压痛指数评分的差异,比较两组治疗后的临床疗效差异。结果:与治疗前相比,两组治疗后及治疗后3个月随访时VAS评分、压痛指数评分均明显下降(P<0.05),且治疗组下降更为显著。治疗组总有效率为97.37%,对照组为83.78%,治疗组疗效优于对照组,差异有统计学意义(P<0.05)。结论:密集型银质针导热疗法治疗肩胛部慢性软组织损伤,能明显改善患者的疼痛症状,且不良反应少,与药物治疗相比,密集型银质针导热疗法在远期疗效方面更具优势。Objective:To observe the clinical effect of intensive silver needle heat conduction therapy on chronic soft tissue injury of scapula.Methods:78 patients with chronic soft tissue injury of scapula were randomly divided into treatment group and control group,with 39 cases in each group.The treatment group was treated with intensive silver needle heat conduction therapy for the tenderness in the scapular area(3 inches apart from the spine,4 lines and 3 rows of needles were placed outwards with a needle distance of about 1.5cm.Each line had 3 silver needles.The needles were kept for 20min,once a week),while the control group was treated with flurbiprofen gel plaster externally(8h/time,once/d,consecutive a week).Both groups were treated for 1 week and then rested for 1 week,continuous treatment for 2 weeks was a course,and continuous treatment for 2 courses.The differences of visual analogue scale(VAS)and tenderness index scores before and after treatment and 3 months after treatment were observed between the two groups,and the differences of clinical efficacy after treatment were compared between the two groups.Results:Compared with before treatment,the VAS score and tenderness index score were significantly decreased after treatment and 3 months follow-up in both groups(P<0.05),and the treatment group showed a more significant decrease.The total effective rate was 97.37%in the treatment group and 83.78%in the control group,the curative effect of treatment group was better than control group,and the difference was statistically significant(P<0.05).Conclusion:Intensive silver needle heat conduction therapy for chronic soft tissue injury of scapula can obviously improve the pain symptoms of patients,and less adverse reactions.Compared with drug therapy intensive silver needle thermal therapy has more advantages in long-term efficacy.
