摘 要:目的测定重庆市万州区2018年—2021年8岁~10岁学生的碘营养水平,为其制订防治措施提供依据。方法根据地理方位,每年在万州区抽取5所小学,每所小学抽取40人,年龄8岁~10岁,共抽取学生833人,收集其家庭食用盐与尿样用于碘含量检测;其中2019年和2021年学生开展B超甲状腺检查。结果2018年—2021年盐样检测共833份,其中合格碘盐789份,合格碘盐食用率为94.7%。2018年—2021年尿样检测共833份,尿碘中位数分别为238.1μg/L、193.9μg/L、207.5μg/L、242.8μg/L。不同年份学生尿碘中位数比较,差异有统计学意义(H=20.075,P<0.05)。不同地区学生尿碘中位数比较,城区学生(252.1μg/L)高于乡镇学生(209.1μg/L)(Z=-4.732,P<0.05)。2019年和2021年学生甲状腺B超检查共414人,其甲状腺肿大率为1.7%。结论万州区2018年—2021年8岁~10岁学生总体碘营养充足;城区学生尿碘中位数高于乡镇学生,应多关注城区学生,避免碘过量。Objective To test the iodine nutrition level among students aged 8-10 in Wanzhou District from 2018 to 2021,and provide basis for the formulation of prevention measures.Methods Five primary schools were selected according to the geographical location in Wanzhou District every year,and about 40 students aged 8 to 10 were selected from each school.The household salt and urine samples were collected for iodine content detection in 833 students.The students'thyroid was measured by B-ultrasound in 2019 and 2021.Results There were 789 qualified iodized salt samples in 833 salt samples which were tested from 2018 to 2021,and the consumption rate of qualified iodized salt was 94.7%.A total of 833 urine samples were tested from 2018 to 2021,and the median urinary iodine were 238.1μg/L,193.9μg/L,207.5μg/L and 242.8μg/L,respectively.There was statistical significance on the difference in the median urinary iodine of students in different years(H=20.075,P<0.05).Comparison of the median urinary iodine among students in different regions showed that it was higher in the urban students(252.1μg/L)than in the township students(209.1μg/L)(Z=-4.732,P<0.05).The incidence rate of goiter was 1.7%in 414 students in 2019 and 2021.Conclusion The overall level of iodine nutrition among the students aged 8 to 10 in Wanzhou District from 2018 to 2021 is sufficient.The median urinary iodine of urban students is higher than that of township students,and more attention should be paid to urban students to avoid iodine intake excessive.
