摘  要:目的探究糖尿病肾病(DN)尿毒症患者尿路感染血清微小RNA-15a(miRNA-15a)、微小RNA-34b(miRNA-34b)、微小RNA-103(miRNA-103)及细胞色素P4501A2(CYP1A2)基因多态性的相关性。方法选取2018年2月-2020年8月在郑州市第七人民医院接受治疗的DN尿毒症患者82例,根据其是否发生尿路感染分为感染组22例与非感染组60例,检测对比两组患者的微小RNA(miRNA)、CYP1A2等血清表达情况。结果感染组患者的糖尿病病程、导尿管滞留时间长于未感染组(P<0.05),血清C-反应蛋白(CRP)、白细胞介素-6(IL-6)水平高于未感染组(P<0.05)。感染组患者的血清miRNA-15a表达水平低于未感染组(P<0.05),miRNA-34b、miRNA-103表达高于未感染组(P<0.05)。CYP1A2基因检测结果显示,两组患者的rs2069514-3859位点基因型存在统计学差异(P<0.05),感染组等位基因A、AA基因型分布高于未感染组(P<0.05)。Logistic回归分析显示,血清CRP、miRNA-15a、miRNA-34b、miRNA-103水平与CYP1A2基因型rs2069514-3859位点AA基因或等位基因A是尿路感染的独立影响因素(P<0.05)。结论DN尿毒症患者的血清miRNA-15a、miRNA-34b和miRNA-103表达以及炎症因子CRP参与尿路感染过程,同时携带CYP1A2基因型rs2069514-3859位点A等位基因更易发生尿路感染,临床中可结合以上指标评估DN尿毒症患者发生尿路感染的可能性,及时采取预防措施,改善患者预后。OBJECTIVE To explore the correlation of serum miRNA-15a,miRNA-34b and miRNA-103 with cytochrome P4501A2(CYP1A2)gene polymorphism in patients with diabetic nephropathy(DN)uremia and urinary tract infection.METHODS A total of 82 patients with DN uremia treated in Zhengzhou Seventh People′s Hospital from Feb 2018 to Aug 2020 were selected.According to whether they had urinary tract infection,they were divided into infection group(22 cases)and uninfected group(60 cases).The expression of miRNAs and CYP1A2 was compared between the two groups.RESULTS The course of diabetes and catheter indwelling time of the infected group were significantly longer than those of the uninfected group(P<0.05),and serum CRP and IL-6 levels were significantly higher than those in the uninfected group(P<0.05).The expression level of serum miRNA-15a in the infected group was significantly lower than that in the uninfected group(P<0.05),and the expression levels of miRNA-34b and miRNA-103 were significantly higher than those in the uninfected group(P<0.05).The detection results of CYP1A2 gene showed that the genotypes of rs2069514-3859 were different in the two groups(P<0.05),and the frequencies of allele A and AA genotype in the infected group were significantly higher than those in the uninfected group(P<0.05).Logistic regression analysis showed that serum CRP,miRNA-15a,miRNA-34b,miRNA-103 levels and CYP1A2 genotype rs2069514-3859 AA gene or allele A were independent influencing factors of urinary tract infection(P<0.05).CONCLUSION The expression of serum miRNA-15a,miRNA-34b and miRNA-103 in patients with DN uremia and the inflammatory factor CRP are involved in the process of urinary tract infection.Meanwhile,patients carrying allele A at CYP1A2 rs2069514-3859 are more prone to urinary tract infection.In clinical practice,the above indicators can be used to evaluate the possibility of urinary tract infection in patients with DN uremia,so as to help take preventive measures in time,thereby improving the prognosis.
