摘 要:目的探究肠道菌群失调介导的细菌移位感染及p38丝裂原活化蛋白激酶(p38 MAPK)信号通路对子痫前期的影响。方法选择2018年1月-2020年1月于医院进行剖宫产分娩的122例子痫前期孕妇(研究组)和110名健康孕妇(对照组),采集所有孕妇粪便标本,通过16S rRNA高通量测序技术分析两组孕妇肠道菌群结构多样性,采用免疫组化法观察胎盘组织中p38 MAPK表达情况,采用实时荧光定量聚合酶链式反应(RT-PCR)法检测胎盘组织中p38 MAPK mRNA表达水平,并采用酶联免疫吸附测定(ELISA)法检测外周血可溶性血管内皮生长因子受体1(sFlt1)、可溶性内皮糖蛋白(sEng)水平。结果研究组Chao 1、OTUs低于对照组(P<0.05),Shannon、PD-whole-tree与对照组比较,差异无统计学意义;研究组放线菌门、拟杆菌门、疣微菌门比例低于对照组(P<0.05),变形菌门比例高于对照组(P<0.05),厚壁菌门与对照组比较,差异无统计学意义;研究组小杆菌属、梭状芽孢杆菌、韦荣氏球菌属比例高于对照组(P<0.05),毛螺菌属、阿克曼氏菌、粪杆菌属比例低于对照组(P<0.05);研究组胎盘组织中p38 MAPK阳性表达率及p38 MAPK mRNA水平均高于对照组(P<0.05);研究组外周血sFlt1、sEng水平均高于对照组(P<0.05)。结论肠道菌群失调介导的细菌移位感染及p38 MAPK信号通路激活均可能参与了子痫前期的发病过程。OBJECTIVE To explore the effects of bacterial translocation infection mediated by intestinal flora imbalance and p38 MAPK signaling pathway on preeclampsia.METHODS Total of 122 pregnant women with preeclampsia(study group)and 110 healthy pregnant women(control group)who delivered by cesarean section in the hospital between Jan 2018 and Jan 2020 were recruited.Stool specimens of all pregnant women were collected,and high throughput sequencing of 16S rRNA was used to analyze the diversity of intestinal flora between the two groups of pregnant women.The expression of p38 MAPK in placental tissue was observed by immunohistochemistry,and the mRNA expression level of p38 MAPK in placental tissue was detected by real-time quantitative polymerase chain reaction(RT-PCR).ELISA was used to detect the levels of soluble vascular endothelial growth factor receptor 1(sFlt1)and soluble endoglin(sEng)in peripheral blood.RESULTS Chao 1 and OTUs in the study group were lower than those in the control group(P<0.05),and there were no significant differences in the Shannon and PD-whole-tree between the two groups.The proportions of Actinobacteria,Bacteroides and Verrucomicrobia were lower in the study group than those in the control group(P<0.05),while the proportion of Proteobacteria was higher than that in the control group(P<0.05),but there was no statistically significant difference in Firmicutes when compared to the control group.The proportions of Dialister,Clostridium,Veillonella and Fusobacterium in the study group were higher than those in the control group(P<0.05),while the proportions of Lachnospira,Akkermansia and Fecal bacillus were lower than those in the control group(P<0.05).The positive expression rate and mRNA level of p38 MAPK in placenta tissue of the study group were higher than those of the control group(P<0.05).The levels of peripheral blood sFlt1 and sEng in the study group were higher than those in the control group(P<0.05).CONCLUSION The bacterial translocation infection mediated by intestinal flora imbalance
