摘 要:目的探究补肾活血复方对慢性心衰(CHF)大鼠肠道水液代谢与结肠核因子-κB(NF-κB)、闭合蛋白(Occludin)的相关性影响。方法将60只SD雄性大鼠随机分为空白组15只和实验组45只。实验组大鼠通过结扎冠脉左前降支联合力竭式游泳、饥饿,建立慢性心衰模型。成模的大鼠随机分3组,即模型组、托伐普坦组及补肾活血组。托伐普坦组灌胃托伐普坦混悬液1.35mg/(kg·d),补肾活血组灌胃补肾活血中药汤剂15.75g/(kg·d),空白组和模型组灌胃等体积蒸馏水。灌胃28日后,采用酶联免疫法(ELISA)检测大鼠血清氨基末端脑钠肽前体(NT-proBNP),心脏彩超检测大鼠左心室结构与功能,免疫印迹法(Western blotting)检测大鼠结肠组织NF-κB、Occludin蛋白表达,苏木素-伊红(HE)染色观察大鼠心肌与结肠组织。结果与空白组比较,模型组大鼠LVEdd、LVEsd、NT-proBNP升高,LVEF、LVFS降低,结肠组织NF-κB蛋白表达升高,Occludin蛋白表达降低(P<0.05)。与模型组比较,补肾活血组大鼠LVEdd、LVEsd、NT-proBNP降低,LVEF、LVFS升高,结肠组织NF-κB蛋白表达降低,Occludin蛋白表达升高(P<0.05)。结论补肾活血复方可能通过下调CHF大鼠结肠组织NF-κB蛋白表达,上调紧密连接蛋白Occludin表达,改善肠黏膜屏障损伤,调节水液代谢紊乱,起到治疗CHF作用。Objective To explore the effect of Bushen Huoxue Decoction on the correlation between intestinal fluid metabolism and colonic nuclear factor-kappaB(NF-κB)and Occludin in rats with chronic heart failure.Methods 60 male SD rats were randomly divided into 15 rats in the blank group and 45 rats in the experimental group.The rats in the experimental group established the model of chronic heart failure by ligating the left anterior descending coronary artery combined with exhaustive swimming and starvation.The successful modeling rats were randomly divided into three groups:model group,tovaptan group and Bushen Huoxue group.Tovaptan group was gavaged with 1.35mg/(kg·d)of tovaptan suspension,Bushen Huoxue group was gavaged with 15.75g/(kg·d)of Bushen Huoxue traditional Chinese medicine decoction,and the blank group and model group were gavaged with equal volume of distilled water.after gavaged 28 days,the serum N-terminal pro-brain natriuretic peptide(NT-proBNP)was detected by enzyme-linked immunosorbent assay,the structure and function of left ventricle were detected by cardiac color Doppler ultrasound,the expressions of NF-κB and Occludin proteins in rat colon were detected by Western blotting,rat myocardium and colon was observed by hematoxylin eosin staining.Results Compared with the blank group,LVEDd,LVESD and NT-proBNP increased、LVEF and LVFS decreased,NF-κB protein expression increased,and Occludin protein expression decreased in the model group(P<0.05).Compared with the model group,LVEDd,LVESD and NT-proBNP decreased,LVEF and LVFS increased,NF-κB protein expression decreased,and Occludin protein expression increased in tovaptan group and Bushen Huoxue group(P<0.05).Conclusion Bushen Huoxue decoction may play a role in the treatment of CHF by downregulating the expression of NF-κB protein and up regulating the expression of tight junction proteins Occludin,improving the injury of intestinal mucosal barrier and regulating the disorder of water metabolism.
