血清CA199辅助早期诊断肝吸虫感染及预后评估的应用研究

摘　　要：目的探讨血清糖类抗原199(CA199)辅助早期诊断肝吸虫感染及预后评估的应用价值。方法选取46例无症状肝吸虫感染患者作为感染组,其中30例患者服药治疗后作为疗效评估组;另选取54例健康体检者作为对照组。三组均进行血清CA199水平检测。比较三组血清CA199水平,分析血清CA199水平与无症状肝吸虫感染相关性及血清CA199水平在无症状肝吸虫感染中的诊断价值。结果感染组、疗效评估组、对照组血清CA199水平分别为(56.4±17.9)、(36.3±11.6)、(20.7±7.8)kU/L。三组血清CA199水平比较差异有统计学意义(P<0.05);感染组和疗效评估组血清CA199水平明显高于对照组,感染组血清CA199水平明显高于疗效评估组,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示,血清CA199水平与无症状肝吸虫感染呈正相关(r=0.378,P=0.001<0.05)。CA199预测无症状肝吸虫感染的曲线下面积(AUC)为0.824[95%CI=(0.627,0.887)],临界值为33.6 kU/L,特异性为77.4%,敏感性为40.6%。结论血清CA199水平与无症状肝吸虫感染具有显著相关性,对无症状肝吸虫感染具有较高的评估价值。Objective To discuss the application value of serum carbohydrate antigen 199(CA199)in the early diagnosis and prognosis assessment of liver fluke infection.Methods 46 patients with asymptomatic liver fluke infection were selected as the infection group,of which 30 patients were selected as the efficacy evaluation group after drug treatment.Another 54 healthy subjects were selected as control group.Serum CA199 levels were detected in all three groups.Serum CA199 levels were tested in all three groups.The serum CA199 levels of the three groups were compared,and the correlation between serum CA199 levels and asymptomatic liver fluke infection,as well as the diagnostic value of serum CA199 levels in asymptomatic liver fluke infection were analyzed.Results Serum CA199 levels in infection group,efficacy evaluation group and control group were(56.4±17.9),(36.3±11.6)and(20.7±7.8)kU/L,respectively.There was statistically significant difference in serum CA199 level among the three groups(P<0.05).The serum CA199 level of the infection group and the efficacy evaluation group was significantly higher than that of control group,and the serum CA199 level of the infection group was significantly higher than that of the efficacy evaluation group.The differences were statistically significant(P<0.05).Pearson correlation analysis showed that serum CA199 level was positively correlated with asymptomatic liver fluke infection(r=0.378,P=0.001<0.05).The area under the curve(AUC)of CA199 for predicting asymptomatic liver fluke infection was 0.824[95%CI=(0.627,0.887)],with a critical value of 33.6 kU/L,a specificity of 77.4%and a sensitivity of 40.6%.Conclusion Serum CA199 level is significantly correlated with asymptomatic liver fluke infection and has high value in evaluating asymptomatic liver fluke infection.
