曼月乐和安宫黄体酮治疗围绝经期异常子宫出血的效果及对子宫内膜厚度的影响分析

摘　　要：目的探究左炔诺孕酮宫内节育系统(商品名:曼月乐)和安宫黄体酮治疗围绝经期异常子宫出血的效果及对子宫内膜厚度的影响。方法68例围绝经期异常子宫出血患者,按照电脑随机法分为观察组和对照组,每组34例。观察组患者实施曼月乐治疗,对照组患者实施安宫黄体酮治疗。对比两组患者的治疗效果、不良反应发生情况及治疗前后性激素指标[雌二醇(E2)、卵泡刺激素(FSH)、促黄体生成素(LH)]、月经情况、子宫内膜厚度。结果观察组治疗总有效率为97.06%(33/34),高于对照组的58.82%(20/34),差异具有统计学意义(P<0.05)。治疗后,观察组E2(227.52±2.85)pmol/L、FSH(10.05±1.22)U/L、LH(9.22±1.52)U/L均低于对照组的(305.52±3.52)pmol/L、(12.52±1.89)U/L、(10.52±1.88)U/L,差异均有统计学意义(P<0.05)。治疗后,两组月经量少于本组治疗前,月经时间短于本组治疗前,子宫内膜厚度低于本组治疗前,且观察组月经量(42.02±1.02)ml少于对照组的(60.25±2.45)ml,月经时间(4.02±1.02)d短于对照组的(5.79±1.88)d,子宫内膜厚度(6.25±1.02)mm低于对照组的(8.45±1.45)mm,差异具有统计学意义(P<0.05)。两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论对围绝经期异常子宫出血患者实施曼月乐治疗,能够降低子宫内膜厚度,改善性激素水平,且不良反应少,值得临床研究和推广。Objective To investigate the effect of levonorgestrel intrauterine system(trade name:Mirena)and medroxyprogesterone in perimenopausal patients with abnormal uterine bleeding and its influence on endometrial thickness.Methods A total of 68 patients with perimenopausal abnormal uterine bleeding were randomly divided into observation group and control group,with 34 cases in each group.The observation group was treated with Mirena,and the control group was treated with medroxyprogesterone.Both groups were compared in terms of therapeutic effect,occurrence of adverse reactions,sex hormone indexes[estradiol(E2),follicle stimulating hormone(FSH),luteinizing hormone(LH)],menstruation and endometrial thickness.Results The total effective rate in the observation group was 97.06%(33/34),which was higher than that of 58.82%(20/34)in the control group,and the difference was statistically significant(P<0.05).After treatment,E2 of(227.52±2.85)pmol/L,FSH of(10.05±1.22)U/L,and LH of(9.22±1.52)U/L in the observation group were lower than those of(305.52±3.52)pmol/L,(12.52±1.89)U/L,and(10.52±1.88)U/L in the control group,and the differences were statistically significant(P<0.05).After treatment,the menstrual volume in the two groups was less than that before treatment in this group,the duration of menstruation was shorter than before treatment in this group,and the endometrial thickness was lower than that before treatment in this group;the menstrual volume of(42.02±1.02)ml in the observation group was less than that of(60.25±2.45)ml in the control group;the duration of menstruation of(4.02±1.02)d in the observation group was shorter than that of(5.79±1.88)d in the control group;the endometrial thickness of(6.25±1.02)mm in the observation group was lower than that of(8.45±1.45)mm in the control group;the differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Mirena can reduce the endometrial thic
