丙泊酚Wada试验对语言、记忆和运动功能术前评价与治疗的价值

摘　　要：目的:探讨丙泊酚Wada试验对语言、记忆力和运动功能的术前评价与治疗的价值。方法:选取2020年1月—2022年9月广西壮族自治区脑科医院行术前丙泊酚Wada试验的患者12例作为研究对象。统计患者丙泊酚Wada试验结果,监测患者不良反应,观察术前、术后记忆力等功能变化,评估患者癫痫发作与认知能力。结果:12例患者均完成术前双侧丙泊酚Wada试验,优势语言侧在左侧患者10例(83.33%),优势语言侧在右侧患者2例(16.67%);左侧记忆优势患者11例(91.67%),右侧记忆优势侧患者1例(8.33%);肌力正常患者10例(83.33%)。12例患者丙泊酚静脉注射后,出现头痛眼痛患者1例,出现震颤伴有面部扭曲患者1例,不良反应发生率为16.67%(2/12);患者术后6个月癫痫发作频率、持续时间低于术前1周,差异有统计学意义(P=0.008)。结论:丙泊酚Wada试验可有效明确患者术前语言、记忆和运动功能状态,术前评价与治疗资料可提供较高的价值。Objective:To explore the value of propofol Wada test for preoperative evaluation and treatment of language,memory and motor function.Methods:Twelve patients who underwent propofol Wada test before operation in the Guangxi Zhuang Autonomous Region Brain Hospital from January 2020 to September 2022 were selected as the study subjects.Adverse reaction of the patients was monitored.The changes of memory and other functions before and after the operation were observed.The epileptic seizures and cognitive ability were evaluated.Results:All 12 patients completed the bilateral propofol Wada test before operation,10 patients(83.33%)had the dominant language lateralization on the left side,and 2 patients(16.67%)had the dominant language lateralization on the right side.There were 11 patients(91.67%)with memory dominance on the left side and 1 patient(8.33%)with memory dominance on the right side.There were 10 patients(83.33%)with normal muscle strength.After 12 patients received intravenous propofol injection,1 patient developed headache and eye pain,1 patient developed tremor with facial distortion,the incidence of adverse reactions was 16.67%(2/12).The frequency and duration of epileptic seizures at 6 months after surgery were lower than those at 1 week before surgery,and the difference was statistically significant(P=0.008).Conclusion:Propofol Wada test can effectively determine the preoperative language,memory and motor function status of patients,and preoperative evaluation and treatment data can provide high value.
