摘 要:目的:探讨无锡市职业性铅接触人群职业危害及健康促进干预效果。方法:选取2020年1月-2022年6月无锡市某蓄电池厂职业性铅接触工人500名为观察组,另选取该厂未从事铅作业的工人300名为对照组。根据是否进行健康促进干预将观察组分为健康促进组280名和非健康促进组220名。比较各组工人血铅、血常规、肝功能检测结果。结果:观察组工人血铅异常率、血红蛋白(Hb)降低率、血小板(PLT)降低率、网织红细胞(RET)升高率高于对照组,差异有统计学意义(P<0.05);观察组工人丙氨酸氨基转移酶(ALT)升高率、天冬氨酸氨基转移酶(AST)升高率高于对照组,差异有统计学意义(P=0.000);健康促进组工人血铅异常率、Hb降低率、PLT降低率、RET升高率低于非健康促进组,差异有统计学意义(P<0.05);健康促进组工人ALT升高率、AST升高率低于非健康促进组,差异有统计学意义(P<0.05)。结论:职业性铅接触人群存在血常规、肝功能异常情况,推行健康促进干预可调节血铅异常、血常规异常、肝功能异常情况。Objective:To explore the occupational hazards and health promotion intervention effects of occupational lead exposed population in Wuxi city.Methods:From January 2020 to June 2022,a total of 500 occupational lead exposed workers in a storage battery factory in Wuxi city were selected as the observation group,and 300 non-lead exposed workers in this factory were selected as the control group.The observation group was divided into health promotion group(280 cases)and non-health promotion group(220 cases)according to whether health promotion intervention was performed.The results of blood lead,blood routine test and liver function test of workers in each group were compared.Results:The rate of abnormal blood lead,the rate of decreased hemoglobin(Hb),the rate of decreased platelet(PLT),and the rate of increased reticulocyte(RET)in the workers of observation group were higher than those in the control group,and the difference was statistically significant(P<0.05);the rate of increased alanine aminotransferase(ALT)and the rate of increased aspartate aminotransferase(AST)in the workers of observation group were higher than those in the control group,and the difference was statistically significant(P=0.000).The rate of abnormal blood lead,rate of decreased Hb,rate of decreased PLT and rate of increased RET in the health promotion group were lower than those in the non-health promotion group,and the difference was statistically significant(P<0.05).The rates of increased ALT and increased AST in the health promotion group were lower than those in the non-health promotion group,and the difference was statistically significant(P<0.05).Conclusion:Abnormal blood routine and liver function are present in the occupational lead exposed population.Health promotion intervention can regulate abnormalities in blood lead,blood routine and liver function.
