摘 要:目的通过与超声心动图检查对比,评估256层iCT左心室收缩功能分析与超声左心室收缩功能分析的相关性,并探讨256层iCT简化4时相Simpson法测量左心室收缩功能的可行性。资料与方法回顾性分析河北医科大学第四医院2014年12月—2015年12月72例拟诊断为冠心病或术前排除心脏疾病的肿瘤患者的冠状动脉CT血管造影和超声心动图资料。冠状动脉CT血管造影检查后常规自动重建出0%、40%、45%、75%共4个时相的心脏容积数据。随后在操作台重建出另外16个时相数据,累计获得以5%R-R间期(0%~95%)为重建间隔的共20个时相心脏容积数据。采用Simpson法分别获得4时相、20时相左心室每搏量(LVSV)、左心室射血分数(LVEF)、左心室心输出量(LVCO)。评价4时相、20时相Simpson法与超声心动图检查结果的相关性,分析4时相与20时相Simpson法测量结果的一致性。结果①4时相、20时相Simpson法测得LVEF、LVSV、LVCO结果与超声心动图检查结果呈高度正相关(r=0.938~0.999,P<0.001)。②4时相与20时相Simpson法测量LVEF、LVSV、LVCO结果呈高度正相关(r=0.999,P<0.001)。Bland-Altman分析显示4时相与20时相Simpson法测量结果具有极好的一致性。72例患者中,68例(94.44%)4时相与20时相Simpson法测量结果完全相同。③4时相Simpson法重建分析时间明显短于20时相Simpson法(t=−157.439,P<0.001)。结论256层iCT左心室收缩功能分析与超声左心室收缩功能分析的相关性极好。256层iCT简化4时相Simpson法测量左心室收缩功能结果与20时相Simpson法测量结果具有极好的相关性和一致性,并且能够明显缩短重建分析时间。Purpose To evaluate the correlation of left ventricular systolic function between 256-slice iCT and ultrasound by comparing with echocardiographic results,and to explore the feasibility of simplified 4-phase based on Simpson method to analyze the left ventricular function via 256-slice iCT.Materials and Methods Seventy-two patients with clinically proposed diagnosis of coronary artery disease between December 2014 and December 2015 in Hebei Medical University Fourth Affiliated Hospital were enrolled.Cardiac volume data of 4 phases at 0%,40%,45%,and 75%were routinely and automatically reconstructed at the end of coronary CT angiography for all patients.Subsequently,additional 16 phases data were reconstructed at the control station,and a total of 20 phases with 5%R-R interval(0%-95%)as the reconstruction interval were obtained cumulatively.Left ventricular stroke volume(LVSV),ejection fraction(LVEF),and cardiac output(LVCO)were obtained via the Simpson method,respectively.All the left ventricular function parameters obtained from the two data sets were analyzed via Pearson analysis and consistency analysis was performed.CT cardiac function parameters were compared with the echocardiographic data between the two groups,respectively.Results①LVEF,LVSV and LVCO measured by the 4-phase based on Simpson method and the 20-phase based on Simpson method were correlated extremely well with echocardiography results(r=0.938-0.999,P<0.001),respectively.②Of the 72 patients,68(94.44%)had the same results for the 4-phase Simpson method as for the 20-phase Simpson method.LVEF,LVSV and LVCO obtained from the 4-phase Simpson method showed an excellent positive correlation(r=0.999,P<0.001)with those measured with the 20-phase Simpson method.③The analysis time of reconstruction based on the 4-phase Simpson method was significantly shorter than that of the 20-phase Simpson method(t=−157.439,P<0.001).Conclusion The left ventricular systolic function measured by simplified 4-phase reconstruction based on Simpson method of 256-sli
